Kabuki syndrome is a rare condition characterized by intellectual disability, poly-malformative s... more Kabuki syndrome is a rare condition characterized by intellectual disability, poly-malformative syndrome, and distinctive facial dysmorphia. It also exhibits clinical and biological heterogeneity, with rare and diverse symptoms. Genetic analysis plays a significant role in both positive diagnosis and prognosis. Recently, whole exome sequencing has identified several genes responsible for the disease, notably KMT2D and KDM6A. Studying new mutations in this disease will contribute to understanding the role of these genes in the pathogenesis of Kabuki syndrome. We report a case of a 9-and-a-halfyear-old boy, born to non-consanguineous parents, diagnosed with Kabuki syndrome. This article describes the prenatal diagnosis process and the postnatal progression. Genetic analysis revealed a novel missense point mutation in the KMT2D gene, classified as a class 4 missense mutation, responsible for Kabuki syndrome.

Background: The use of information and communication technology such as telemedicine occupies now... more Background: The use of information and communication technology such as telemedicine occupies nowadays a huge place in modern medicine practice all over the world, mainly after the COVID-19 pandemic. However, its implementation in Tunisia and other developing countries has achieved little success with low utilization and can be challenging for several reasons. In this study, our aim was to assess the knowledge, attitudes and practice of Tunisian medical doctors regarding telemedicine. Methods: This was a cross-sectional web survey, administered to medical doctors in Tunisia in October 2022. Respondents' level of knowledge of telemedicine was assessed by calculating a knowledge score (0 to 12). Attitude subsections were about perceived telemedicine attributes of relative advantage, compatibility, trial ability and complexity. Results: A total of 243 participants were included. The mean age was 45 ± 9.6 years old, and 57.2% were female, with a mean of 14.3 ± 10.3 years of professional experience. The majority (95.9%) had an average or high level of computer skills. More than half (59.3%) had a poor level of telemedicine knowledge. A good level of knowledge was significantly associated with age category over 50 years (p = 0.02) and with years of experience over 10 (p = 0.03). The majority (89.3%) had a moderate or high score about perceived advantages. The majority (88.5%) accepted use of telemedicine in their future practice. Almost half (46.9%) had practiced telemedicine activities before using a mobile phone (91%) or social media (64%). The principal limitations of applying telemedicine were challenges of organization and implementation, and incomplete patient examination. Conclusions: Although Tunisian doctors' knowledge and practice of telemedicine were unsatisfactory, their positive attitude and willingness to try it in their future practice were encouraging. The patient presented initially with abdominal pain with, fever, bleeding and pelvic mass on ultrasonography leading to the wrong diagnosis of a pelvic abscess. Dilation and curettage were performed and pathological examination confirmed the diagnosis. of placental site trophoblastic tumor. Therefore, she underwent abdominal hysterectomy. Four years after surgery, the patient is still disease free. Gestational trophoblastic diseases should be considered in every patient presenting abnormal uterine bleeding after delivery or pregnancy loss despite the associated symptoms being very unusual.

According to the World Health Organisation (WHO), the global prevalence of EBF during the first six months of life remains low particularly in low-and middle-income countries. In this systematic review (SR) and meta-analysis (MA) we examine the existing evidence of the pooled prevalence and determinants of EBF during the first 6 months of infants' lives in North African (NA) countries. Methods: This SR and MA followed the Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA) guidelines. International databases were systematically searched. Inclusion criteria included studies conducted among mothers in NA countries (Tunisia, Algeria, Morocco, Libya, Egypt, Sudan) on breastfeeding prevalence. Metaanalyses were conducted using R (version 4.2.3). Results: 16 studies with a total of 7,849 participants were included in the analysis. Objectives To assess the effectiveness of Triclosan coated suture in reducing surgical site infec... more Objectives To assess the effectiveness of Triclosan coated suture in reducing surgical site infections (SSIs) rate after caesarian delivery (CD). Study design Three hundred eighty patients were randomly assigned to closure with polyglactin non coated suture VICRYL, or with polyglactin coated suture VICRYL Plus after caesarian section. The primary outcome was the rate of SSIs within 30 days after surgery and secondary outcomes were the rate of wound healing complications. Results SSI rate was 2.5% in Triclosan group compared to 8.1% with non-coated suture. Use of Triclosan coated suture (TCS) was associated with 69% reduction in SSI rate (p = 0.037; ORa:0.294; 95% CI:0.094-0.921). The use of Triclosan coated suture was associated with statistically lower risk of wound oedema (2.5% vs 10%), (p = 0.019; OR:0.595), dehiscence (3.8% vs 10.6%), (p = 0.023; OR:0.316) and hematoma (p = 0.035; OR:0.423). Conclusion Our results confirm the effectiveness of Triclosan coated suture in reducing SSI rate and wound healing disturbances. Clinicopathological data on pathological tumor size, the status of pathological lymph node metastasis, and clinical course were extracted from patients' medical records. Histological slides were reviewed for variables including tumor morphology and hormonal status. Additional clinical data were obtained from electronic medical records. The Kaplan-Meier method was used to determine the association between survival and TILS. Results Our series contained 123 cases of invasive ductal carcinomas. The mean age was 52 years with extremities of 26 and 102 years. TILS were not significantly associated to response to neoadjuvant chemotherapy (p= 0,728), to metastases (p= 0,737), neither to recurrences (p=0,939). Furthermore, TILs were not associated with the overall survival (p=0,928). Conclusions In this series, TILs seem not to be associated with outcomes. We did not find additional benefits for estimating TILs in triple-positive breast cancer.
