Inguinal exploration was carried out and showed common cord. Biopsy was sent and identified the presence of sperm in left testis only.

BACKGROUND The postpartum period is critical, and women are at highest risk for perinatal complic... more BACKGROUND The postpartum period is critical, and women are at highest risk for perinatal complications, yet patient attendance at postpartum clinic visits is low. OBJECTIVE To determine if decreasing the time to initial postpartum (PP) visit from six weeks to two weeks can increase attendance rates at PP visits. STUDY DESIGN We conducted a parallel, randomized, non-blinded trial at a publicly insured clinic comparing a single six-week postpartum visit (control) to two visits at two and six weeks postpartum (intervention). The primary outcome was attendance at one or more routine PP visit. Secondary outcomes were emergency department (ED) visits within 30 days of delivery and non-routine clinic visits. Multivariable regression was performed to identify predictors of clinic non-attendance. To demonstrate a significant increase from the baseline clinic attendance rate of 70% to 85%, 250 participants were needed. RESULTS Between November 2018 and March 2020, 250 patients were randomized and analyzed. The patient population had multiple comorbidities, notably obesity (53%), diabetes (30%), mental health disorders (22%) and hypertensive disorders (21%). The attendance at one or more PP visit was not significantly different among the control and intervention arms (58% vs 70%, p=0.065). The 2-week visit had an attendance rate of 41% (51/125) and the 6-week visit had an attendance rate of 60% (151/250). After adjusting for confounders, significant predictors of PP visit non-attendance included younger age, multiparity and being a patient from the high-risk obstetric clinic. ED usage was similar among the control and intervention arm (8% vs 6%, p=0.635). However, more patients in the control arm utilized the clinic for non-routine visits (30% vs 16%, p=0.010). In response to a patient satisfaction survey on the optimal timing of the postpartum visit, the majority (59%) of respondents would have preferred both the 2- and 6-week visit. CONCLUSION The addition of a two week postpartum visit to the six week postpartum visit did not increase the likelihood of attendance at a routine visit but did decrease the amount of urgent clinic visits.

As the mechanism for carbon dioxide production is not fully understood, it is possible that oesophageal intubation may also generate a capnograph. Our aim was to measure ETCO2 levels following (1) oesophageal and (2) tracheal intubation in fresh-frozen cadavers, and to observe the size, shape and duration of any capnographic waveform. Four fresh frozen cadavers underwent oesophageal intubation by an emergency medicine specialist with confirmation by a second specialist. Hand ventilation with room air via a self-inflating resuscitation bag was provided at 12 breaths per minute for 2 min or until ETCO2 was zero for 10 consecutive breaths. Competent performance of cricothyroidotomy, lateral canthotomy and resuscitative thoracostomy is ... more Competent performance of cricothyroidotomy, lateral canthotomy and resuscitative thoracostomy is an expected standard for Australasian emergency physicians, but infrequent exposure to these procedures could impair physician confidence, reducing the likelihood of their execution in a critical timeframe. Training to perform these procedures is a recognised challenge for non-surgeons, and cadaver-based training is one method of addressing this need. We describe a 1 day cadaver-based workshop for emergency medicine doctors and briefly report on its impact on physician confidence. This workshop appeared effective in increasing the confidence of emergency medicine physicians to carry out rarely performed life and sight-saving procedures and also provides an opportunity for senior clinicians to increase compliance with continuing profession development schemes.

A substantial amount of commentary has been dedicated to George W. Bush's policy towards Iraq, wi... more A substantial amount of commentary has been dedicated to George W. Bush's policy towards Iraq, with many perceiving it as a departure from traditional US foreign policy practice. The objective of this research is to address and challenge this contention. This thesis takes 1979 as the launching point from which to examine this assertion, as it is both the year in which Saddam Hussein gained power as president of Iraq and also the year when the United States began tilting favourably towards, and reviving its relations with, this country. To embark on this study a descriptive and analytical narrative of the evolution of US foreign policy towards Iraq is presented, ending with the George W. Bush terms of office. This study contends that US policy towards Iraq has two major dimensions: the first is US policy towards Arab Iraq and the second its de facto policy towards Iraqi Kurdistan; both are defined by US strategy at the supra-national level. To guide this study, continuity and change were used as guiding concepts for analysing US Iraq policy at the national and subnational levels. The study argues that US Iraq policy is primarily one of continuity rather than change, as US national interests and security, as regards Iraq, are defined by the same fundamental concerns, regardless of the nature of the American administration in office. The research concludes that, from the inception of the Bush administration, and the attacks on 9/11 that followed, the actual invasion of Iraq and US actions in its aftermath were all guided by the same considerations and goals. iii Declaration No material in this thesis has been previously submitted for a degree at this or any other University. There was patchy involvement elsewhere with brownish desquamation in places (see Fig. 1(a) and (b)). The whole appearance was markedly different from her initial presentation to her GP. The infected skin was not itchy, but painful. The erythema and pain had gradually increased during the previous few weeks. The discomfort was so severe that she was admitted for pain relief.

Presently, we analyzed the regulation of CA9 expression during hypoxia by HIF-1α, Notch3, and the von Hippel-Lindau (VHL) in breast carcinoma cells. Both HIF-1α and Notch3 were absolutely required for the expression of CA9 mRNA, protein, and reporter. Reciprocal co-immunoprecipitation of HIF-1α, Notch3 intracellular domain (NICD3), and pVHL demonstrated their association. The presence of common consensus prolyl hydroxylation and pVHL binding motifs (L(XY)LAP);LLPLAP(2191) suggested an oxygen-dependent regulation for NICD3. However, unlike the HIF-1α protein, NICD3 protein levels were not modulated with hypoxia or hypoxia-mimetic agents.
