Objectives: To assess the prediabetes and associated risk factors in medical students of Bangladesh. Materials and Methods: A cross-sectional study was done among 252 medical students of Shaheed Ziaur Rahman Medical College, Bogura. Data related to demography with some behavioral factors, anthropometry, blood pressure, fasting plasma glucose, family history of hypertension (HTN) and diabetes mellitus (DM) were collected. Prediabetes was assessed by the American Diabetes Association (ADA) and World Health Organization (WHO) criteria. Results: The proportion of prediabetes was 13% according to ADA and 1.6% according to WHO respectively. Students with a positive family history of hypertension (HTN) had more prediabetes compared to those who didn't have. None of the demographic factors, family history of DM or HTN, exercise and obesity were related between prediabetic and normal students, though prediabetic students had more proportion of family history of HTN. Conclusion: Prediabetes is proportionately high among students with positive family history of HTN. We recommend a regular checking of fasting plasma glucose of medical students with special attention to those who have a strong positive family history of DM and HTN.

Typically, these are non-caseating epithelioid granulomata involving organized collections of activated macrophages and T lymphocytes. In countries where tuberculosis is endemic, sarcoidosis is often misdiagnosed and mistreated as tuberculosis. We present case report of a 47-year-old female who presented with 2 years history of recurrent, multiple nodules with occasional joint pain & fever and had received anti-tubercular therapy without any improvement. A diagnosis of sarcoidosis is made finally and she was managed accordingly.

Demographic and Clinical Characteristics of Patients with Primary Hypothyroidism and Adequacy of Levothyroxine Replacement Therapy in Them: A Nationwide Study in Bangladesh, 2024
Background: Despite the huge burden of primary hypothyroidism in Bangladesh, no nationwide study ... more Background: Despite the huge burden of primary hypothyroidism in Bangladesh, no nationwide study has been conducted describing the demographic and clinical characteristics of these patients; data on the adequacy of levothyroxine (L-T4) replacement are also lacking. This study aimed to fill the knowledge gap. Materials and Methods: This cross-sectional study was conducted among patients with primary hypothyroidism attending the endocrinology outpatient clinics of several hospitals throughout Bangladesh from July 2023 to December 2023. Adults with primary hypothyroidism who had been on L-T4 replacement for at least 1 year were included. Data on their demographic and disease-related characteristics were collected. Serum thyroid-stimulating hormone (TSH) was measured; TSH 0.4-4.0 µIU/mL was considered the therapeutic target for nonmalignant thyroid cases. Results: Two thousand six hundred and forty-one patients (median age 37 years, 86.4% female, and median disease duration 5 years) were included. Hashimoto's thyroiditis was the most common (40.9%) cause of hypothyroidism; 1.9% had differentiated thyroid cancer (DTC), and the etiology was not established in many (40.2%). Only 26.8% of patients had high adherence to L-T4. Among the non-DTC cases (n = 2590), over half (53.9%) had TSH within the target range, whether the disease was uncontrolled in the remaining 46.1% (TSH was below the target range in 5.6% and above the target range in 40.5%). Among the DTC cases (n = 51), only a few (9.8%) had a TSH within the target range; TSH was below and above the range in 2% and 88.2% of these patients, respectively. In the nonmalignant hypothyroid cases, vegetarian diet, smokeless tobacco use, disease duration >5 years, iron or calcium supplements consumption within 4 h of L-T4 intake, and low or medium adherence to L-T4 were associated with uncontrolled hypothyroidism. Conclusion: For nearly half of Bangladeshi patients with primary hypothyroidism, L-T4 replacement is not optimal. Physicians should address the barriers to reaching the therapeutic goal for these patients.

Background: Although the frequencies of getting drug naive type 2 diabetes patients among all the... more Background: Although the frequencies of getting drug naive type 2 diabetes patients among all the diabetic patients are very low, nowadays it claims more attention in the treatment procedures of drug naive diabetic patients. But in Bangladesh, we have very few research-oriented data regarding the demographic, clinical and biochemical characteristics of drug naive type 2 diabetes patients. The aim of this study was to determine the demographic clinical and biochemical characteristics of drug naive type 2 diabetes patients of Bangladesh. Methods: This was an open label observational real-life study which was conducted in the chambers of the investigators in several places of Bangladesh as outdoor setting during the period from August 2020 to December 2020. In total, 250 patients with drug naive type 2 diabetes mellitus were enrolled as the study population. Proper written consents were taken from all the participants before starting data collection. A pre-designed questionnaire was used in patient data collection. All data were processed, analyzed and disseminated by MS Office and SPSS version as per need.
