Background: Acute otitis externa, an inflammatory condition of the external auditory canal, is a ... more Background: Acute otitis externa, an inflammatory condition of the external auditory canal, is a common clinical problem in general medicine. Objectives: This study aimed to determine the etiology of otitis externa in patients from the Mazandaran province, north of Iran, which has a humid climate, as humidity can affect the prevalence of pathogenic microorganisms. Patients and Methods: This cross-sectional study involved 116 patients with otitis externa. Two sets of samples were collected from their ears; one set was used for slide preparations, and the other for microbial culturing. After culturing, the microorganisms were identified by conventional methods. Results: Patients between 35 and 44 years of age were most frequently affected (25.00%) by otitis externa (average age, 43.87 ± 18.08 years). Moreover, women (54.31%) were more frequently affected than men (45.69%). Upon direct investigation, Gram-positive bacilli were the most commonly identified microorganisms (22.41%). Furthermore, Bacillus spp. and coagulase-negative staphylococci (22.41% and 19.83%, respectively), were the organisms most frequently identified from cultures of otitis externa samples. Conclusions: Direct examination and culture showed that a mixed infection of fungi and bacteria is the most common cause of otitis externa. The present study revealed that Bacilli spp. were the most abundant bacteria isolated from patients with otitis externa. Thus, it is recommended that both organisms should be considered as etiologic agents in protocols for treatment of otitis externa.

Domperidone is a prokinetic drug that increases gastrointestinal motility. Objectives: The aim of this study was to evaluate the effect of domperidone on the treatment of FAP in children. Methods: In this double-blind clinical trial study, FAP was diagnosed in 80 children aged 5 - 14 years, who were referred to Amirkola Children's Hospital in Babol for one year based on the criteria of the Rome IV. Then, they were randomly divided into two groups of 40 patients. Group A received domperidone tablets (0.25 mg/kg, three-time/day) for two months, and group B received a placebo. Objectives: Different drugs are prescribed for the induction of ovulation in patients with polycy... more Objectives: Different drugs are prescribed for the induction of ovulation in patients with polycystic ovary syndrome (PCOS). The aim of this study was to compare the effect of letrozole alone with letrozole plus N-acetyl cysteine (NAC) on the pregnancy rate in patients with PCOS. Materials and Methods: This clinical trial studyincluded317 patients with PCOS in the infertility clinic of Zahedan in 2018. Patients were randomly divided into letrozole and NAC plus letrozole groups. Then, several parameters were evaluated in both groups, including the pregnancy rate, number of follicles larger than 18 mm, endometrial thickness, and the endometrial pattern. Finally, data were analyzed using SPSS 25, and t-test and chi-square tests were used for statistical analysis. The mean age of study participants was 28.5 ± 4.9 years old. The mean duration of infertility in the intervention group was 4.5 ± 3.8 years. The mean anti-Müllerian hormone and the mean endometrial thickness of patients in the intervention group were 4.4± 3.6 ng/mL and 7.6 ± 2.5 mm, respectively. Eventually, the pregnancy rate was 14.6% (n = 23) and 7.5% (n = 12) in the intervention and control groups, respectively (P = 0.046). The results of this search revealed that the number of follicles with the size of 18 mm in the control group was higher compared to the intervention group, but the fertility rate in patients with PCOS receiving letrozole plus N-acetylcysteine was significantly higher.

Background and Objective: Elective cesarean section (ECS) increases neonatal respiratory complica... more Background and Objective: Elective cesarean section (ECS) increases neonatal respiratory complications like transient tachypnea of the newborn (TTN) and respiratory distress syndrome (RDS). This pilot study examined the effect of antenatal steroids on the prevention of respiratory problems in full-term neonates born via ECS. Methods: This experimental study was carried out on full-term neonates (39-42 weeks) born by ECS to the mothers admitted to Babol Clinic Hospital, northern, Iran in 2016. The intervention group received betamethasone (12 mg, intramuscular, once a day) for 2 days before ECS plus conventional care, but the control group received only conventional care. The rate of respiratory complication and the admission rate of the newborn ward and neonatal intensive care unit (NICU) were compared between two groups. Findings: Overall, 200 full-term neonates (100 neonates in the experimental group and 100 neonates in the control group) were enrolled. Nine neonates (9%) in the experimental group and 8 neonates (8%) in the control group had TTN (P=0. 64), and one (1%) neonate in the experimental group and one neonate (1%) in the control group had RDS (P=1). Conclusion: Antenatal corticosteroid administration to the mothers before ECS with gestational ages of 39-42 weeks does not reduce the incidence of respiratory complications. Thus, further studies are needed to determined its effects in gestational age group more than 39 weeks.

Background and Aim: Urinary tract infection (UTI) is one of the most common infections in childre... more Background and Aim: Urinary tract infection (UTI) is one of the most common infections in children. Arbitrary use of antibiotics has increased the resistant strains and transmits these resistant factors to other pathogens. Therefore, this study was designed to follow the resistance trend in 9-years in children with urinary tract infections referred to the Amirkola Children's hospital (Northern Iran). In this cross-sectional study, all children who had Urinary tract infections between 1994 to1998 and 2013 to 2018 with one month to 18 years old were included. Data (covering 9 years), such as age, sex, antibiotic resistance profile, urine culture results, and history of UTI, were collected retrospectively from patients' records. Cases were selected based on clinical and microbiological criteria. Antimicrobial susceptibility was determined by the Kirby-Bauer disk diffusion method. Results: Escherichia coli was the most predominant organism isolated from urine samples. The highest drug resistance was related to cefazolin (83.3%). Resistance to nitrofurantoin was higher in children older than 5 years (P≤0.001). Moreover, the resistance pattern to gentamicin, amikacin, ceftriaxone, cephalexin, and imipenem differed in females and males (P>0.05). A 9-year study showed that 91.4% of the strains are still sensitive against amikacin which is a suitable treatment option in the studied strains. The establishment of antimicrobial stewardship programs and regular monitoring of antimicrobial resistance could help to reduce inappropriate prescribing for UTIs.

A medical worker is one of the most important occupational groups at risk for the virus. Despite vaccination, in many of them, antibody reduced and if the exposure to infected materials, there is a possibility of infection to HBV. This study, examines some of hepatitis B serologic markers among students in Babol University of Medical Sciences. Materials and Methods: 236 students in Babol University of Medical Sciences were enrolled to our study. For all students participating in the study, a questionnaire including demographic information, history of the first and last dose vaccination for Hepatitis B, blood transfusion and blood products and surgical history were completed. 5 ml of venous blood was taken from each student and serum was separated and stored in -20 oC. For all samples, ELISA test operated for Anti-HBs and Anti-HBc and HBsAg.

Human papilloma virus is suspected of causing extragenital cancers, including: cancers of the oral cavity, larynx, oesophagus and the lungs. More than 40 genotypes of HPV that infect the anogenital area are associated with a large spectrum of diseases from benign proliferation to invasive cancers. The aims of our study were to evaluate the prevalence of HPV-infection in non-cancer oesophageal lesions in Mazandran, North of Iran, and identify the prevalence of HPV in benign lesions. Methods: A total of 104 non-cancer oesophageal samples were collected in paraffinated blocks of tissue archived in pathology. After deparaffinization and DNA extraction, nested polymerase chain reaction (nPCR) method was performed by HPV L1 primer pairs (MY09/MY11 and Gp5+/GP6+). Results: Of 104 cases, 35 (33.7%) were HPV DNA positive. By histopathology, 61.5%, 31.8%, 30.4% and 30% of dysplasia, oesophagitis, normal tissue and ulcerative lesions were positive for HPV DNA, respectively. Also, the lower-third (distal) of the oesophagus had less infection of HPV DNA (18.8%) than the upper and middle-third (about 46%). The highest prevalence of HPV DNA was found in cases > 75 years old. Conclusion: To our knowledge, considering the highest rate of HPV-infection in people more than 75 years, we recommend the diagnostic procedures at a lower age. Also, the proximal part of the oesophagus is more infected with HPV than the distal part.
