Carbonic anhydrase (CA) is a zinc metalloenzyme widely distributed throughout the tissues of the ... more Carbonic anhydrase (CA) is a zinc metalloenzyme widely distributed throughout the tissues of the body. In the kidney, this enzyme is thought to play a pivotal role in urinary acidification and bicarbonate reabsorption. Two distinct isozymes of carbonic anhydrase have now been identified in the mammalian kidney. A soluble cytoplasmic form, similar if not identical to human erythrocyte carbonic anhydrase C, accounts for the bulk of the renal carbonic anhydrase activity. In addition, a membrane-bound form constituting only about 2-5% of the renal activity has been found in the brush border and basolateral fractions of kidney homogenates. The activity of carbonic anhydrase enzyme is incessantly, which means a high energy expenditure, but ATP is not the source of energy of this enzyme (CA). The kidney generates 180 liters of filtrate a day. The process is also known as hydrostatic filtration due to the wrong belief that 180 liters of filtrate a day as result of the hydrostatic pressure exerted, by the left ventricle; on the capillary walls of the kidney. However, the cardiac pressure required to impel the blood stream at adequate level is way beyond 120 mm Hg output force that left ventricle has.

Methods: In a case series of 11 patients who presented with post-rhinoplasty abscess or skin tract, we try to consider the interval, etiology, risk factors, age, and gender to mention the possibility of the presence of skin tract as a post-rhinoplastic sequel. Results: In 11 patients (4 females,7 males) we discovered 5 cases of skin tract that probably occurred due to remaining suture. Conclusion: The allergic history of patients should be considered and the suturing method and kind of material should be noticed. Positive habitual history should be considered before surgery in case selection and the smoker patients have to be advised to reduce or quit smoking.

Background: Anti-Müllerian hormone (AMH) is widely recognised as the gold standard for assessing ... more Background: Anti-Müllerian hormone (AMH) is widely recognised as the gold standard for assessing ovarian reserve. It is also utilised to predict ovarian response during controlled ovarian stimulation in assisted reproductive technology (ART) and is key indicator of oocyte yield, embryo quality, embryo euploidy, and live birth rates. Methods: This multicentre retrospective study analysed the prevalence of AMH levels in women aged 18-50 years who attended Fakih IVF Fertility Centres (Abu Dhabi, Al Ain, and Dubai) in the UAE for their initial fertility consultation between 2017 and 2023. AMH levels were classified as normal (≥1.3 ng/mL) or low (<1.3 ng/mL). Results: A total of 8,533 women underwent AMH testing, with 63.62% having normal AMH levels and 36.38% showing low AMH levels. No significant differences in AMH levels were observed across demographic groups. From 2017 to 2021, normal AMH levels remained stable at around 63-65%, but in 2022-2023, they declined to 58-59%, indicating a 5-7% increase in the number of patients with low AMH. This rise was particularly notable, with a 4.3-8.3% increase in women under 30 and a 7.7-9.5% increase in those aged 31-40. Conclusion: This study reveals a decline in AMH levels across all age groups during 2022-2023 compared to the previous five years, with a more significant decrease noted in women under 40. Given AMH's role as a key predictor of successful ART outcomes, early intervention for women under 40 years particularly those under 35, may be crucial for improving ART success. Further research with a larger sample size is necessary to confirm these findings.

As a prevalent malignancy with substantial incidence and mortality worldwide, head and neck squam... more As a prevalent malignancy with substantial incidence and mortality worldwide, head and neck squamous cell carcinoma (HNSCC) originates from multiple locations of different tissues, which level up the difficulty and complexity of the cancer detection. The diagnosis requires highresolution imaging modalities and several approaches have been developed within recent decades to provide detailed anatomical information for HNSCC. This review aims to summarize basic knowledge of those mainstream modalities including ultrasound, magnetic resonance imaging (MRI), computed tomography (CT) and positron emission tomography (PET). Moreover, novel and advanced imaging techniques, such as radiomics and dual-energy computed tomography (DECT), will be described as newly developed approaches of quantitative and personalized medicine. By comparing the advantages and disadvantages of these techniques, we would like to additionally discuss the directions of next generation imaging approaches.

A 29yrs old woman from Obowo in Imo State Nigeria presented with a mild abdominal cramp and inter... more A 29yrs old woman from Obowo in Imo State Nigeria presented with a mild abdominal cramp and intermittent diarrhoea in one of the health facilities in Owerri. She had been discharging tapeworm segments since 2020 from her anus and sometimes these were found around her vagina without any major discomfort. Her husband had been away during this period and was due to return home. This made her decide to visit the health facility to present this situation so as not to possibly infect her husband. Sample containers were given to her to collect her stool samples, urine and any worm segments daily for 5days. A total of ten samples (both stool and urine) were analyzed using parasitological procedures. The recovered segments were examined using histological procedures for proper identification. Praziquantel and Metronidazole was administered to the patient and a follow-up stool analysis was carried out on 14th and 28th day after treatment. Eighteen worm segments were recovered from stool, anal region and around her vagina respectively. Out of the eighteen worm segments recovered, 4(22.0%) were from stool sample, 11(61.1%) from anus, 3(16.7%) around the vagina and none were recovered from urine samples. They were all identified as proglottids of Taenia saginata specie. Cyst of Entamoeba coli was also isolated in all the stool samples while few eggs of Taenia specie was detected in two out of five stool samples examined. Segments were not recovered in the urine samples and the Scolex of the tapeworm was not recovered in this study. The results of the post treatment analysis of both stool and urine samples were negative for Taenia saginata and Entamoeba coli respectively. The discharge of the segments from the anus stopped completely. There is need to enlighten the public on the importance of consuming properly cooked meat (Beef) and to maintain a good environmental sanitation to prevent Taeniasis and other parasitic infection.

Background: A radicular cyst (RC), also known as a periapical cyst, is the most common type of od... more Background: A radicular cyst (RC), also known as a periapical cyst, is the most common type of odontogenic cyst. It typically develops at the apex of a tooth's root due to chronic inflammation following pulp necrosis, often resulting from untreated dental caries or trauma. A radicular cyst is commonly found in anterior maxilla, not commonly seen in the mandible. The bilateral symmetrical presentation of this cyst is quite rare. This case report emphasizes the surgical role in the early diagnosis and treatment of such lesions and the overall oral rehabilitation of such mutilated dentition. Case Report: This report describes the case of a 60-yearold healthy African male who presented with a rare case of bilateral symmetrical radicular cyst in the mandibular premolar region along with advanced periodontal disease and hopeless remaining dentition. The patient management comprised surgical enucleation of the cystic sac for histopathologic examination, extraction of associated remaining roots, and total mouth extraction under local anesthesia followed by bone augmentation and implantsupported overdenture prosthesis. This case was followed up at 6, 12, 24 and 36 months. Radiographic analyses revealed complete healing at the sites of the cyst's enucleation with no signs of recurrence or preapical pathology. Conclusions: This case presented a common condition in an uncommon age group, sex, location, and presentation. It highlights the importance of clinical and radiographic diagnosis and reinforces the role of histopathologic examination. This case also demonstrates the scope of dental management and comprehensive care of a dental patient with a hopeless dentition.

Background : Pulmonary hypertension (PH) is a common complication of heart failure (HF) with a si... more Background : Pulmonary hypertension (PH) is a common complication of heart failure (HF) with a significant impact on disease progression, and mortality. Left ventricular assist device (LVAD) implantation can improve haemodynamic status and survival as a "bridge to transplantation". Aim of this study is to characterise haemodynamic after LVAD implantation based on the latest definitions of the PH Guidelines 2022, and to identify predictors of haemodynamic changes. Methods : Patients with advanced HF with reduced ejection fraction (HFrEF) implanted with LVAD between 2011 and 2021 were retrospectively reviewed. Variables obtained by right heart catheterisation (RHC) were compared between baseline (T1) and after LVAD implantation (T2). Difference between pulmonary vascular resistance (∆PVR) at T1 and T2 was analysed by linear regression. Results : Of the 100 patients with LVAD implantation, 37 patients were selected for the analysis. Mean age was 49 ± 13 years and 76 % were male. Thirty-three out of 37 patients (91.7%) had PH at baseline, whose 27 patients (75%) with combined post-capillary and pre-capillary (Cpc) PH. At T2, PH was observed in 20 out of 37 patients (54.1%) whose 6 patients (16.7%) with Cpc-PH. Significant reductions in mean pulmonary arterial pressure (22 ± 8 mmHg, ∆ =-14 mmHg, p <0.001) and pulmonary vascular resistance (2.3 ± 1.1 WU, ∆ =-1.0 WU, p <0.001) were observed. Background : Chronic traumatic encephalopathy (CTE) is a progressive neurodegeneration that can b... more Background : Chronic traumatic encephalopathy (CTE) is a progressive neurodegeneration that can be widely distributed throughout the brain due to accumulation of neuropathologic lesions following exposure to repetitive head impact (RHI) causing concussions or sub-concussive traumatic brain injury (TBI). Populations at risk include military personnel and contact sports athletes. A neuropathological severity staging system has been developed based on lesion complexity. Aim : To systematically review the clinicopathologic correlations and neuroimmunologic mechanisms in CTE. Methods : We conducted a systematic review by literature search through WorldCat.org and ProQuest Central, inclusive of PubMed and EBSCOhost electronic databases, to Index, supportive treatments for associated Post-Traumatic Stress Disorder (PTSD), and education. Further research could identify targetable molecular mechanisms, quantify traumatic load, and test pharmacologic treatment strategies.

Studying the interaction of proteins and genes by current computational methods is effective in n... more Studying the interaction of proteins and genes by current computational methods is effective in new drug design. Considering the special role of lactoferrin (Lf) identifying its action mechanism is a new gate to the targeted treatment of some diseases. This study aimed to investigate the molecular interaction of Lf with virus spike, the bacterial cell binding receptor, and the proteins involved in apoptosis by in silico research. The docking results showed that the amino acids involved in the interaction between the spike virus of COVID-19 with the ACE2 receptor were similar to the amino acids in the interaction between the virus spike and Lf, and the binding between Lf and the viral spike was more inclined. The N-terminus of Lf interacted with the N-terminus region of the CD14, which contains the binding sites of bacterial lipopolysaccharide to the cell. Lf can compete for binding with bacterial lipopolysaccharides. The results related to the docking of TP53, BAX, and BAK1 gene promoters and Lf showed that Lf with one or more amino acids that are responsible for binding to DNA interacted with TP53 and BAX genes with higher binding affinity. In all protein and protein-gene interactions, the N-lobe region of Lf, which is its functional region, is involved in these interactions, and the anticancer, antibacterial, and antiviral, roles of Lf are related to this region. Lf is important as a medicinal supplement for treating COVID-19 and cancer.

Background/aim: Hysterectomy is the most common operation performed for gynecologic reasons. In t... more Background/aim: Hysterectomy is the most common operation performed for gynecologic reasons. In this study, we aimed to investigate unexpected malignancies in patients who underwent hysterectomy for benign reasons. Materials and methods: In this retrospective study, pathology results of hysterectomy cases performed in our clinic between 2013 and 2023 were analyzed and unexpected malignancy results were reported. Results: A total of 1954 patients underwent hysterectomy between 2013 and 2013. The mean age of the patients was 50.31 years and the most common indications for hysterectomy were myoma uteri (48.41%) and abnormal uterine bleeding (38.41%). Patients with no suspicion of malignancy in preoperative imaging and endometrial sampling results were operated and 21 patients with malignant final pathology results were identified. The mean age of these patients was 55.47 years and 13 of them had endometium cancer and 7 of them had ovarian malignancy. Conclusion: In the literature, the rate of unexpected malignancies in hysterectomy cases was 0.73%, of which approximately half (58.3%) were uterine and the other half ovarian. In our study, the rate of endometrial malignancies was 0.66% and ovarian malignancies were 0.30%. Although malignancies can be excluded by preoperative imaging methods and endometrial sampling, tumors are rarely encountered.

Contextual pain conditioning is the process of associating a specific painful experience to its e... more Contextual pain conditioning is the process of associating a specific painful experience to its emotional and environmental context. Although pain conditioning is suggested to play a key role in the development and maintenance of chronic pain, there is still paucity of studies and protocols specifically devoted to better understanding its underlying mechanisms. The present study offers an ecological experimental model of contextual aversive (painful and non-painful) conditioning using immersive virtual reality (iVR) technology. The protocol comprised both context and sensory stimuli, specifically: i) a three-roomed virtual apartment presented in iVR, ii) a painful aversive electro-stimulation (PA), and iii) non-painful aversive sounds (NPA). Acquisition and extinction phases consisted in participants completing ten rounds in the apartment, for which each room was associated to a specific stimulation delivery (PA, NPA or no stimulation for control). Sympathetic skin response (SSR), environment valence perception and stimulation evaluation were measured throughout the entire experiment to assess behavioral and physiological markers of aversive conditioning. Results confirmed that both aversive conditionings lead to a decrease in associated room valence compared to control, even more so for the PA associated room, which resettled after extinction. Together with a significantly greater SSR found during anticipation of pain in contrast to both aversive sounds and control, these outcomes may suggest that pain conditioning acquisition was stronger than non-painful aversive conditioning. Concluding modulation of aversive conditioning markers enabled to validate this protocol, which future application may be adjusted to allow studies in chronic pain patients.

This study aims to explore the potential of microfluidic systems for the rapid and sensitive dete... more This study aims to explore the potential of microfluidic systems for the rapid and sensitive detection of herpes viruses in clinical contexts. Microfluidic systems are smallscale devices that allow accurate control of liquids and can perform complex interactions that conventional diagnostic methods can observe. The study reviewed the existing literature and discussed the advantages, limitations, and landscape of the design of microfluidic devices capable of detecting a wide range of herpes viruses with high sensitivity and specificity. The practical difficulties associated with the manufacture, optimization, and validation of devices are also taken into account. The results show that microfluidic systems are capable of detecting Herpes viruses quickly and accurately at the nursing home, thereby eliminating the need for complex laboratory equipment and specialized training. Regarding the design of devices with broad detection capabilities, challenges remain in the construction and validation of technical problems. Despite these challenges, microfluidic systems can revolutionize herpes virus infection diagnosis, especially in resource-limited environments. Continuous development of microfluidic technology is expected to promote the development of highly sensitive, specific, and affordable devices for detecting herpes viruses and other pathogens.

Conclusion: To improve environmental sustainability, this study recommends the development of zoned waste treatment facilities, environmental education programs, and frameworks for cross-border cooperation. The research provides a strategic plan and hopes to offer recommendations for responsive, accountable waste management, particularly in the development of zoned waste treatment facilities. The waste management zoning is divided into three areas: coastal settlements, port areas, and mangrove conservation areas. In the Conclusion, the study suggests strategies to improve waste management, such as:Developing zoned waste treatment facilities, Implementing environmental education programs,Establishing cross-border cooperation between Indonesia and Malaysia to address waste issues effectively.

We present 6 high grade ovarian carcinomas that were associated with seromucinous borderline tumo... more We present 6 high grade ovarian carcinomas that were associated with seromucinous borderline tumors. Patients ranged in age from 26 to 66 years. All cancers appeared as solid regions within otherwise paucicystic tumors that measured from 4.2 cm to 14.0 cm in greatest dimension. Five of the 6 tumors were bilateral, and endometriosis was seen in 4 of the tumors. Clinical data, with follow up periods ranging from 0 to 40 months, was available for all patients. Four patients died of their disease and two had recurrent disease at the time of this review. All tumors exhibited a low-grade carcinoma component, which is consistent with ovarian cancers developing in a progressive adenoma to carcinoma sequence (so-called type I ovarian cancers). Two of the associated low-grade/borderline tumors had focal serous characteristics with WT1+ epithelial cell clustering. High-grade tumors exhibited an array of cell types expressing endometrioid, undifferentiated, serous, clear cell, and mucinous features that were at times mixed within the same tumor. Five cancers were focally WT1+; two showed admixed lowgrade serous features and one showed a high-grade serous adenocarcinoma with transitional features. Two cancers had mucinous features (one with focal CK20 and CDX2 immunohistochemical staining and one only with focal CDX2 immunohistochemical staining). All the high-grade tumors showed a mutated-pattern of p53 nuclear staining with >75% 2-3+ staining. We conclude that seromucinous borderline tumors need to be extensively sampled. Cyst wall membrane rolls are important to evaluate since they may reveal persisting endometriosis and solid regions most likely harbor a high-grade tumor. Identifying high-grade cancer within a seromucinous borderline tumor is important because, although seromucinous borderline tumors and their oftassociated low-grade endometrioid cancers have favorable outcomes, tumors harboring a high-grade component are aggressive and may eventuate in patient death.
