As these diseases have no specific cause or responsible organism, it is therefore very difficult to distinguish and diagnose and thus stands as a major threat for public health. It is assumed that the reason for these diseases may be from genetic to environmental factors, from stress to idiopathic origin. Pemphigus Vulgaris(PV) is such an autoimmune blistering disease that frequently affects the mucous membrane and skin. Very often it starts from oral cavity and Dentists therefore the first to recognize. This paper describes the case of a patient presenting with a one-year history of painful ulcerated gingiva, tongue, floor of the mouth and even on GIT who is finally diagnosed as having PV spreading to generalized skin. But many carriers do not present any symptoms. Operating on undiagnosed COVID-19 patients may cause exposure of medical staff in ward and theatre.A cross sectional was done on 100 consecutive cases suffering from oral and maxillofacial pathologies from 20th May 2020 to 28th August 2020, recruited from Oral and Maxillofacial Surgery department of Dhaka Dental College Hospital and Square Hospitals Ltd, Dhaka, Bangladesh. Out of 100 cases, 7 were affected (7%) by COVID 19 confirmed by RT-PCR in pre-operative screening.Preoperative COVID 19 screening of asymptomatic patients should be considered to improve patient and provider safety and reduce the cost of resource. Incision biopsy revealed metastatic carcinoma with possible primaries was adrenal gland and kidney. Metastatic adenocarcinoma in left lung and primary mass in right adrenal gland were diagnosed by other relevant investigations. Due to unfavorable general condition, Chemotherapy could not be started. Patient died 20 days after the confirmation of diagnosis & during the course of radiotherapy. Burning Mouth Syndrome (BMS) is characterized by chronic oro-facial pain in the absence of specif... more Burning Mouth Syndrome (BMS) is characterized by chronic oro-facial pain in the absence of specific oral lesions & clinically apparent mucosal alterations. It is more commonly observed in middle aged patients & postmenopausal women. It often affects tongue, cheek, lip, hard & soft palate. Usually symptoms are better observed in morning, worsen during the day and typically subside at night. The condition is multifactorial origin, often idiopathic and its etiopathogenesis remain largely enigmatic. Associated medical conditions may include neurologic and metabolic disorder, gastrointestinal, urogenital as well as drug reactions. BMS are of two types, primary & secondary. Primary BMS is essential or idiopathic where secondary BMS is caused by local, systemic and/or psychological factors. Clinical diagnosis depends on the careful history taking, physical examinations and laboratory findings. Vitamin, Zinc or Hormone replacement therapy has been found to be effective with deficiency of the corresponding factors. The drug therapy with alpha-lipoic acid, capsaicin, clonazepam, benzodiazepines, tricyclic antidepressants, anticonvulsants may be effective in symptomatic treatment of BMS. But the treatment is still unsatisfactory and there is no definitive cure.
Sturge Weber Syndrome (SWS) or Encephalotrigeminal Angiomatosis is specifically a rare non heredi... more Sturge Weber Syndrome (SWS) or Encephalotrigeminal Angiomatosis is specifically a rare non hereditary congenital sporadic disorder of elusive etiology. It belongs to a group of disorders collectively known as the phakomatoses ("motherspot" diseases). It has a vast continuum of cutaneous, neurologic, ophthalmic and oral manifestations. It consists of congenital hamartomatous malformations that may affect the eye, skin, and central nervous system (CNS) at different times, characterized by the combination of venous angiomas of leptomeninges, face, jaws and oral soft tissues. The classic pathognomonic features of disease include angioma of the leptomeninges extending to cerebral cortex with ipsilateral angiomatous lesions, unilateral facial nevus after one division of trigeminal nerve and epileptic convulsions. The most characteristic oral manifestation is represented by gingival hemangiomatous lesion usually restricted to ipsilateral maxilla or mandible. We report a case of Sturge Weber Syndrome with its characteristic oral manifestations and review of relevant prevailing literature.

Little is known about the situation in Bangladesh, where tobacco- and areca nut-related head and neck cancers (HNCs) are the most common cancers in men. We aimed to determine the prevalence of HPV in HNSCC in Bangladesh and to explore the possible value of cell cycle markers in clinical diagnostic settings. One hundred and ninety six archival HNSCC tissue samples were analysed for the presence of HPV DNA. The DNA quality was assured, and then amplified using a nested PCR approach. The typing of HPV was performed by automated DNA sequencing. Cellular markers p53, Cyclin D1 and pRb were tested on all samples by immunohistochemistry (IHC), as well as p16 as a putative surrogate for the detection of HPV. A cross sectional study of 29 cases of oral squamous cell carcinoma with or without cervical lym... more A cross sectional study of 29 cases of oral squamous cell carcinoma with or without cervical lymph node metastasis was done among Bangladeshi patients from January 2006 to December 2007. Majority of the study subjects (34.5%) belonged to the age group of 40-49 years. 58.6% of the study subjects were male, while remaining 41.4% of them were female. 51.7% of the lesions were located in the alveolar ridge where the other common sites were buccal mucosa (27.6%) and retro molar area (13.8%). Half of the study subjects (51.7%) were habituated to betel quid chewing followed by 37.9% and 10.3% were habituated to smoking and betel quid-smoking respectively. Grade I lesions was most prevalent (75.9%) in the study subjects. Majority of cases presented with Stage IV lesions (55.2%). The sensitivity, specificity, positive predictive value, negative predictive value & accuracy of clinical palpation method for determining metastatic cervical lymph nodes were 93.33%, 64.29%, 73.68%, 90% and 79.3%...
Trigeminal neuralgia is a sudden, severe, stabbing, recurrent and usually unilateral pain in the ... more Trigeminal neuralgia is a sudden, severe, stabbing, recurrent and usually unilateral pain in the distribution of one or more branches of the fifth cranial nerve. A 55 years old man, diagnosed case of Trigeminal Neuralgia of the left side of the face was treated with oral carbamazepine with good response. But after that the disease became refractory to the drug. With the titration of dose of carbamazepine, raising up to 1200 mg daily in three equal divided doses added with amitryptylene, we could give relief of pain to patient. The patient is now under observation & we are following him for last 6 months with a good response of the combination drug. It justifies that alteration, titration of carbamazepine and combination with amitryptylene can cause remission of trigeminal neuralgia instead of doing surgical intervention. Oral Histoplasmosis is a deep fungal infection may present as oral ulceration and can be confused... more Oral Histoplasmosis is a deep fungal infection may present as oral ulceration and can be confused with oral mucosal malignancies, tuberculosis and other fungal diseases. It mostly affects individual having immunodeficiencies like tuberculosis and AIDS, but may also affect immunocompetent individuals. Here we presented a case of oral histoplasmosis with previously treated pulmonary tuberculosisand was treated by oral itraconazole. Complete disappearance of oral ulceration was evident following four weeks treatment and the patient is in regular follow up with maintenance therapy for last ten months.
