A case is presented of a male patient that presented with intestinal obstruction in the early pos... more A case is presented of a male patient that presented with intestinal obstruction in the early postoperative period of a transabdominal preperitoneal inguinal repair (TAPP) that was diagnosed and repaired successfully using laparoscopy. Whenever a total extraperitoneal procedure (TEP) cannot be performed, the peritoneal closure of the TAPP should be done water-tight using interrupted stitches of absorbable monofilament sutures.

Background: The hepatic hematoma or rupture appear in 1 of every 100,000 pregnancies. The most co... more Background: The hepatic hematoma or rupture appear in 1 of every 100,000 pregnancies. The most common causes of hepatic hematoma in pregnancy are severe preeclampsia and HELLP syndrome; some predisposing factors are seizures, vomiting, labor, preexistent hepatic disease and trauma. Case: A 33 year old primigravid with a normal 33 week twin pregnancy presented abdominal pain and hypovolemic shock due to spontaneous subcapsular hepatic hematoma; laparoscopy was performed to evaluate the possibility of rupture, which was not found, later emergency cesarean section was carried out followed by hepatic hematoma drainage and abdominal packaging by laparoscopy. After surgery the flow through drainage was too high additionally hemodynamic instability and consumption coagulopathy. Abdominal panangiography was performed without identifying bleeding areas. Intesive care was given to the patient evolving satisfactorily, was discharged 19 days after the event. Seven months later she had laparoscopic cholecystectomy due to acute litiasic colecistitis. Discussion: We found 5 cases in literatura about hepatic hematoma during pregnancy no related to hypertensive disorders of pregnancy; these were related to hepatoma, amebian hepatic abscess, falciform cell anemia, cocaine consumption and molar pregnancy. Conclusion: Hepatics hematomas have high morbidity and mortality so is significant early diagnosis and multidisciplinary approach.

Although the technologic advances in this field are amazing, simple measures may sometimes work even better than very sophisticated instruments. This is true of the reclosable plastic bags with zipper-type closure that we use for retrieving surgical specimens from the abdominal cavity in order to protect it as well as the abdominal wall. The bags are cheap, offer no problems for sterilization, are easy to obtain, and are available in many sizes. They are very simple to handle, making it easy to slip in the specimen and then extract it from the abdominal cavity. We describe our experience with these bags and a technique for manipulating them.

Background: Peritonitis continues to be an important cause of morbidity and mortality and often a... more Background: Peritonitis continues to be an important cause of morbidity and mortality and often an etiologic diagnosis is unclear. To evaluate the efficacy and safety of laparoscopy the authors analyzed their 5-year experience with this modality of treatment. Methods: A review was made of 107 consecutive nonselected laparoscopic procedures performed between October 1990 and November 1995. The diagnosis was established by clinical, laboratory, and imaging findings and confirmed by laparoscopy and/or laparotomy. Results: An etiologic diagnosis was unclear in 35% of the cases and was established in all by laparoscopy; 94 patients (87.9%) were successfully treated by laparoscopy while 13 (12.1%) required conversion. Mortality was 4.6%; 14% had postoperative complications and 7.4% had reoperations. Conclusions: Laparoscopic surgery is safe and very efficient in the diagnosis and treatment of patients with peritonitis. In most instances a definitive treatment can be carried out without conversion and has the additional and wellknown advantages of minimally invasive surgery.

A partial pneumothorax developed in a patient undergoing laparoscopic truncal vagotomy when a sma... more A partial pneumothorax developed in a patient undergoing laparoscopic truncal vagotomy when a small pleural laceration was accidentally produced. Changes in oxygen saturation and PETCO2 were immediately detected by the anesthesiologist and measures were taken to maintain the patient's ventilatory stability. The pleural laceration was repaired laparoscopically, and the pneumothorax was corrected by ventilatory manipulation, avoiding the placement of a chest tube. The procedure was completed uneventfully. Literature about the causes of pneumothorax during laparoscopic procedures as well as preventive and therapy viewed.

ABSTRACT RESUMEN Antecedentes: el hematoma hepático sucede en 1 de cada 100,000 embarazos. La pri... more ABSTRACT RESUMEN Antecedentes: el hematoma hepático sucede en 1 de cada 100,000 embarazos. La principal causa es la preeclampsia severa y el síndrome de HELLP. Otros factores predisponentes son: convulsiones, vómito, trabajo de parto, enfermedad hepática preexistente y traumatismos. Caso: paciente primigesta de 28 años de edad, con embarazo gemelar de 33 semanas, dolor abdominal y choque hipovolémico secundarios a hematoma subcapsular hepático espontáneo. Para evaluar la posible rotura se le realizó una laparoscopia, que descartó la hipótesis; enseguida se efectuó cesárea de urgencia seguida de drenaje y empaquetamiento hepático por laparoscopia. La paciente continuó con alto gasto hemático a través de los drenajes, con inestabilidad hemodinámica y coagulopatía por consumo. Se realizó una angiografía sin encontrar sitios de sangrado. Se aplicó factor VII recombinante y se consiguió la disminución del gasto hemático. La paciente continuó con soporte de terapia intensiva con evolución satisfactoria. Egresó del hospital 19 días después. Siete meses después de la intervención se le realizó colecistectomía laparoscópica por colecistitis litiásica aguda. Discusión: en la bibliografía se encontraron cinco casos de hematoma hepático en el embarazo no relacionados con trastorno hiperten-sivo del embarazo, que se vincularon con: hepatoma, absceso hepático amebiano, anemia de células falciformes, consumo de cocaína y embarazo molar. Conclusion: debido a que los hematomas hepáticos originan gran morbilidad y mortalidad es importante que el diagnóstico sea temprano y la atención médica multidisciplinaria.
