Departemen Patologi Klinik yang baru.
Pada kesempatan ini panitia telah mengumpulkan profil masing – masing

spesialis, dimana terdapat 8 orang Dokter Spesialis Patologi Klinik dengan
Curiculum Vitae berikut abstrak dari penelitiannya, serta 7 orang PPDS baru
dengan Curiculum Vitae pada Departemen Patologi Klinik.
Acara ini bertujuan untuk lebih meningkatkan hubungan silaturahmi dan

rasa kebersamaan diantara keluarga besar Patologi Klinik Fakultas Kedokteran
Universitas Sumatera Utara.

THE COMPARISON OF GLYCATED HAEMOGLOBIN (HbA1c) AND

GLYCATED ALBUMIN (GA) IN DIABETES MELLITUS PATIENTS WITH
REGULER HEMODIALISIS

Abstract
Background: Diabetes Mellitus (DM) diagnosis is based on symptoms and blood

glucose test. There have been many reports about the weakness of HbA1c in
monitoring blood glucose in DM patients with regular hemodialysis (HD) lately.
Usually DM with regular HD have lower than normal HbA1c levels because of the
decrease of erythrocyte age in ESRD patients. As an alternative blood glucose
monitoring in DM patients undergoing regular HD is Glycated albumin (GA).
Aim of the study: To compare blood glucose, HbA1c and GA in DM patient with
regular HD and non HD patients.
Methods: This is cross sectional study, observational analytic method in RSUP

H. Adam Malik Medan, January 2016 to February 2016. Study subjects are DM
patients with regular HD and controls are ESRD DM patients without HD.
Analysis using SPSS with Pearson’s correlation test with significant p< 0.05.
Result: There were 27 patients with regular HD and 25 ESRD DM patients

without HD. There were significant differences of HbA1c levels in regular HD
patients and control (6.01±1.06 vs 9.01±2.79; P(0.001), the GA levels
(17.80±5.97 vs 24.85±14.01; P (0.022), and the ureum levels ( 173.49±69.22 vs
133.97±48.05; P(0.022).
Conclusion: A relative low HbA1c (controlled DM) in fact doesn’t show

controlled glucose. The evidence was GA is relatively high (uncontrolled).

HDL/LDL CHOLESTEROL RATIO IN TYPE 2 DIABETES MELLITUS PATIENTS

ABSTRACT
Background : Dislipidemic often following DM illness, noted that DM and

dislipidemic seen may cause a heavy serious combination to death, patient with
DM 2-4 is more risks to cardiovascular disease. It is indeed too important to know
the risks on one with DM, mainly to cardiovascular complication, one of them is to
assess the risk in HDL/LDL ratio. In knowing the risk of HDL/LDL ratio, then as
expected hold a clinical guidance to administer the therapy of dislipidemic on
patient with DM.
Methods : This study is an observational analytical research with cross-

sectional approach. It was done on Department of Clinical Pathology RSUP H.
Adam Malik Medan, involved 50 samples patient-men as diagnosed DM type-2
for 4 years or more, had already consumed Lipid lowering agent medicine for 1
year or more, not consumed any medicines increasing KGD up or blood lipid
level. It has been conducted a check HbA1C to classify the group of patients into
DM type-2 control (HbA1C ≤ 7%) and DM type-2 non-control (HbA1c > 7%) and
also check HDL, LDL rate and ratio assessment HDL/LDL.
The Results : On a pairing t-test there is no found a significant different

HDL/LDL ratio to both groups (p = 0.235). On a correlation test is found a
significant correlations between the length of suffer from DM with HDL/LDL ration
on group DM type-2 non-control (p = 0.0001), however, it is no found any
significant correlation on group DM type-2 controlled ( p = 0.753).
Conclusion : There is no a significant different of HLD/LDL ratio group DM type-

2 suffer from DM for 4 years or more either on control or non-control which by on
both groups has already applied a lipid lowering agent medicines 1 year or more
length.

COMPARISON OF NEUTROPHIL TO LYMPHOCYTE COUNT RATIO AND C-

ABSTRACT
Introduction:Systemic Inflammatory Response Syndrome (SIRS) is a

nonspecific reaction that could be caused by infection, inflammation, or a
combination of both factors. Bacteremia detection in SIRS patients is a challenge
for clinicians. Neutrophil-lymphocyte count ratio (NLCR) and C-reactive protein
(CRP) could rapidly predict bacteremia.
Method:This is a cross sectional study that was conducted in RSUP.H.Adam

Malik Medan Emergency Room from January 2016 to March 2016. Patients that
met SIRS criteria had their NLCR, CRP and blood culture tested.
Result:Of 48 samples, 21 (43.75%) were male and 27 (56.25%) were female. In

this study, there was significant difference of NLCR from both groups, where
NLCR in SIRS with positive blood culture was higher that SIRS with negative
blood culture (38.16 ± 34.97 vs 9.68 ± 5.26, P = 0.001). And there was significant
difference between CRP in both groups. CRP in SIRS with positive blood culture
was higher than SIRS with negative blood culture. (5.89 ± 3.09 vs 3.15 ± 1.89, P
= 0.001).
Discussion:NLCR is a simple biomarker that can be used to detect the presence

of systemic infections in critically ill patients, in this study there was a significant
difference between NLCR and CRP in SIRS patients with positive and negative
blood culture.
Conclusions and Suggesstion:There is significant difference of NLCR and

CRP in SIRS patients with positive and negative blood culture. RNL and CRP
could be used as a marker for detecting bacteremia in SIRS patients.

ABSTRACT
Introduction
Interleukin-8 is a proinflammatory cytokine that is produced by monocytes,

macrophages, and endothelial cells. IL-8 regulates the migration and activation of
leukocytes, and its level increase promptly within 1–3 hours after infection. The
dominance of proinflammatory cytokines influence will cause the emergence of
severe sepsis and septic shock clinical manifestations. Since the early 1990s,
procalcitonin was first described as a specific sign of bacterial infection, serum
procalcitonin concentrations increased in systemic inflammation, particularly
bacterial infections. Procalcitonin increase as sepsis and has been known as a
marker of infection in severe disease.
Method
This was a cross sectional study with observational analytic method, subject of
study was patients with severe sepsis who were treated at the H.Adam Malik
general Hospital Medan.
Interleukin-8 is checked using ChemWell 9210 series. The procalcitonin

examination used the mini VIDAS BRAHMS. Analysis using SPSS. The
correlation test is Spearman rank test used. With a significant value of p <0.05.
Results
Number of subjects was 39 patients, 20 ( 51.3 % ), are males and 19 ( 48.7 % )

are females. With a mean age of 54.1 ± 17.6 years , the youngest is 19 and the
oldest is 87 years old. The average Interleukin-8 in patients was 1647.76 ±
1660.62 pg / mL, while the average levels of procalcitonin patients was 102.67 ±
80.77 ng / mL. There was no significant correlation between interleukin-8 and
procalcitonin levels with a value of r -0.195 (p=0.234 ).
Conclusion
There is no significant correlation between Interleukin-8 and procalcitonin levels

in severe sepsis pateints. Uniformity in the time of sampling is required for further
research .

EXAMINATION OF SERUM PROCALCITONIN TO ASSIST EARLY

DIAGNOSIS OF URINARY TRACT INFECTION IN PATIENTS WITH ACUTE

Abstract
Introduction
Patients with acute stroke are at increased risk for infection which cause death
and worse outcome. The most common infection in stroke is urinary tract
infection (UTI). Current guidelines do not recommend antibiotic prophylaxis in
stroke patients. However, the gold standard for the diagnosis of urinary tract
infection is urine culture, but it takes more than 48 hours for the result. Hence,
UTI diagnostic methods that can provide quick results are needed. Urynalisis is
often used because it is simple and inexpensive. However, false positive results
are common. Procalcitonin is widely recognized as a marker for bacterial
infections and has been reported as a sensitive marker for detection of UTI.
Method
This study is a diagnostic test, carried out in Haji Adam Malik Central Hospital in
23 patients with acute stroke. Measurement of procalcitonin is done by using a
mini VIDAS BRAHMS PCT with the sandwich principle using Enzyme-Linked
Fluorescent Assay methods. The results were compared with urine culture
Result and Discussion

Levels of procalcitonin as a diagnostic test of urinary tract infections is very good
at the cut-off of 0.05 ng/ml with a sensitivity of 100%, specificity 80%, positive
predictive value 94.7% and negative predictive value of 100%.
Conclusion
Procalcitonin has high sensitivity and specificity to aid in the diagnosis of urinary
tract infections in patients with acute stroke.Procalcitonin can be used as a
marker to assist the diagnosis of UTI in patients with acute stroke.

OVERVIEW OF HEMOSTATIC AND PLATELET AGGREGATION IN PATIENTS

WITH NASOPHARYNGEAL CARCINOMA IN HAJ ADAM MALIK HOSPITAL

ABSTRACT
Introduction
There was evidence to suggest that patients with cancers and other malignancies
may have higher tendency of hypercoagulability and thrombosis. Despite this
fact, information about this tendency in nasopharyngeal carcinoma (NPC) is still
scarce.
Method
12 NPC patients were allocated in this study. Blood samples was obtain from the
median vein, in which 15 ml of blood was added with sodium citrate 3,2 % for
the tests of PT, APTT, TT, D-DIMER and PLATELET AGGREGATION TEST, at
the time before the treatments were started, immediately after the 1st, 2nd and
3rd cycles of chemotherapy and after 1 st radiotherapy. Hypercoagulability is
defined as two or more evidence of PT (INR), ratios of APTT, and TT of < 1.0.
The treatment consists of radiotherapy which was performed in the cycle period
with the radiation dose 5000-7000 cGy, whilst the dose of chemotherapy was
Cisplatin 100mg/m2, 5 FU (Fluorouracil) 1000 mg/m2. Statistical analysis for the
TREND is carried out using the ANNOVA test.
Result and Discussion

PT, APTT, and TT were relatively unchanged during the course of the treatment
(p>0.05) however the PT, APTT and TT showed evidence of hypercoagulability
starting before therapy. D-DIMER is increased (p<0.05), and might explain the
thrombus formation is destroyed. There is hyperaggregation in platelet
aggregation test all the way during radiochemotherapy treatment. (p>0.05)
Conclusion and suggestion

In NPC there is evidence of hypercoagulability and this phenomenons is not
affected by the course of radio-chemotherapy. D-Dimer is increased and might
explain the thrombus formation is destroyed. This finding might explain the
scarcity of report of thrombotic evidence in NPC.
This study is continued until the end of the treatment cycle to find out how far
radiochemoteraphy effects towards hemostasis and platelet aggregation
disorders .

ABSTRACT
BACKGROUND : The risk factor of CAD : Hypercholesterolemia, diabetes,

hypertension, smoking habit, age, sex and genetic. Knowing that risks in CAD
is to count the ratio LDL Cholesterol/ HDL Cholesterol > 5 have the risk 6
times . Aiming this ratio, maintaine the clinical guidnes in CAD therapy. Rate
of Apo B100 can also discribe the risk of CAD 6 times if Apo B 100 ≥ 120
mg/dl.
METHODS : This study is conducted by Crosscectional, and obsevational

analysis in Depatement of Clinical Patology and Cardiology Departement of
Medical School of North Sumatera University. The population are the patients
aidmit to Cardiology Departement with CAD who already had Angiography :
72 persons with stenosis < 70% and stenosis ≥ 70% ( inclution and
eksclution ) and had LDL Cholesterol and HDL Cholesterol and Apo B 100 .
LDL Cholesterol and HDL Cholesterol had been analysed with methode of
Colorimetric enzimatic, Apo B 100 is analysed with imunoturbidimetry.
RESULT : On pairing t-test there is found a significant different LDL/HDL ratio

and rate Apo B 100 to both groups ( p=0.0001). On a correlation test is found
a significant correlation between the stenosis in CAD ( stenosis< 70% and
stenosis ≥ 70% ) with LDL/HDL ratio and rate Apo B100 ( p=0,0001)
CONCLUSION : From this study we gain ratio LDL Cholestrol/HDL

cholesterol and Apo B 100 that can predict the severity of stenosis in CAD.

ABSTRACT
Background: HbA1clevelswerehigher in patientswith diabetesis associated
withincreased risk ofcardiovascular disease. Bilirubinis expected toact asan
endogenous antioxidantandanti-inflammatory. Increasedserumbilirubinwas
associatedwith a reducedrisk ofcoronary heart diseaseandstroke.
Objective: To determine correlation of total bilirubin serum and HbA1c in

controlled and uncontrollable type 2 diabetes mellitus.
Methods: This study is a cross-sectional observational analytic approach that

was performed in RSUP.H.Adam Malik in February – March 2015. A sum of 50
samples were divided into 2 groups: Samples withcontrolled DM (HbA1c<7) and
uncontrolled DM (HbA1c ≥7) amounted to 25 people each. All samples were
examined serum levels of total bilirubin.
We using statistical test unpaired student t test when distribution is normal, and if
the distribution is not normal, we use Mann Whitney test with a significance p
value <0.05
Results: In this study we obtained serum levels of total bilirubin in patients with
HbA1c < 7 and HbA1c ≥ 7 each 0.704 ± 0.299 and 0.560 ± 0.138. Although there
are differences, but we found no correlation between total serum bilirubin levels
and HbA1c in both groups (HbA1c <7, r = 0.119, p = 0.580 and HbA1c ≥ 7, r =
0.234, p = 0.261)
Conclusions: There was no correlation between bilirubin total serum and HbA1c
in both groups
