Moreover, up to 30% of patients who present with early stage disease also relapse and die within 5 years, suggesting the presence of micrometastatic disease at diagnosis. One route of metastatic spread is via the blood stream, hence the detection of tumor cells in blood is likely to provide an important predictive tool with respect to relapse of disease. We have developed a sensitive molecular technique to identify tumor cells in blood using mutations in codon 12 of the K-ras gene as a marker. Detection of circulating tumor cells and micrometastases in patients with cancer should prove use... more Detection of circulating tumor cells and micrometastases in patients with cancer should prove useful in determining prognosis and in planning and monitoring systemic therapies. We have developed immunomagnetic isolation of carcinoma cells followed by reverse transcription polymerase chain reaction (immunobead RT-PCR) as a method for identifying very small numbers of breast cancer cells in blood. The expression of cytokeratin 19 (K19) was used as the marker by which the isolated tumor cells were identified. The immunobead RT-PCR technique allowed detection of one tumor cell per 10(6) leukocytes in whole blood. Immunobead RT-PCR is a highly sensitive method of detecting cancer cells in a hematopoietic environment.

Various methods of management of locally advanced breast cancer have been proposed, including com... more Various methods of management of locally advanced breast cancer have been proposed, including combinations of chemotherapy, surgery, radiotherapy, immunotherapy and hormone manipulation. This retrospective study evaluated the effectiveness of chemotherapy in the management of locally advanced breast cancer in pre- and perimenopausal women by examining the pathology of the mastectomy specimens. Sixteen women who on initial clinical examination had breast cancers measuring 5 cm or greater underwent chemotherapy prior to surgery. Four women were also treated with radiotherapy prior to surgery. All 16 women underwent mastectomy and axillary clearance. All specimens showed residual tumour in the mastectomy specimen or the regional lymph nodes. Chemotherapy is useful in reducing tumour burden to allow surgical resection, but does not produce centripetal shrinkage of tumour, nor sterilize the breast of cancer. The central venous access port device (CVAPD) provides reliable vascular access and is used for d... more The central venous access port device (CVAPD) provides reliable vascular access and is used for delivery of fluids and for obtaining blood samples. However, it can carry some intra- and post-operative complications such as thrombosis, pneumothorax, catheter fracture and port malposition. This article describes one surgeon's experience over a 16-year period and strategies to avoid complications. The data were prospectively collected from 1996 to 2012 (16-year period). Collected data included age and gender of the patient, pathology, type of port used, anaesthetic type, side and site of insertion, and complications. A total of 958 devices were implanted. The average age of the patient was 57.8 years. Sixty-eight complications were recorded with an average complication rate of 7.1%. This rate decreased from 23% between 1996 and 1997 to 3.6% from 2010 to 2013. Venous thrombosis was the main source of complications with an incidence of 2.5%. IMPORTANCE Hospital readmissions are believed to be an indicator of suboptimal care and are the f... more IMPORTANCE Hospital readmissions are believed to be an indicator of suboptimal care and are the focus of efforts by the Centers for Medicare and Medicaid Services to reduce health care cost and improve quality. Strategies to reduce surgical readmissions may be most effective if applied prospectively to patients who are at increased risk for readmission. Hospitals do not currently have the means to identify surgical patients who are at high risk for unplanned rehospitalizations.
Many different anesthetic techniques have been used for extracorporeal shock wave lithotripsy, ea... more Many different anesthetic techniques have been used for extracorporeal shock wave lithotripsy, each with its own benefits and disadvantages. A study was made to determine the efficacy and safety of subarachnoid sufentanil as an analgesic technique for lithotripsy. Eight consecutive patients from the University of Michigan Medical Center and twelve consecutive patients from the University of Vermont College of Medicine were retrospectively studied. The need for additional anesthesia, incidence of adverse effects, and discharge times were assessed. Of the 20 patients, 18 were treated successfully with subarachnoid sufentanil. One patient had inadequate anesthesia. One procedure was postponed owing to intractable pruritus. Adverse effects occurred in four patients and were successfully treated. The mean discharge time was 132 minutes, which is comparable with those of other anesthetic methods. Subarachnoid sufentanil is a safe and effective technique for lithotripsy. A prospective study to compare it with other methods is justified.
