PROBLEM FRAMING
• Water powered servo sticking in open position while in operation. Maintenance applying lubricant to

Problem Identification servo sleeve. Issue returns when lubricant isn’t applied. Is the lubricant returning to the water
penstock? Is there a way to utilize hydraulic control or electrical control? Should there be preventive
maintenance on the water powered servo? Does the Servo have preventive maintenance strategies?

QUESTIONS
1. Describe the steps you would take to investigate the fault.
a) Visit the site, if possible, to gather data and evidence. Perform data analysis of historical data to gain insight into the problems and its failure modes. Check
for preventive maintenance strategies on the servo and in the functional locations around the system
b) Produce a logic tree to track down the root cause and recommendations of solving the problem.
c) Depending on the outcome of the RCM analysis, we either conduct a full-blown RCA or not.
d) If the outcome is preventive maintenance, I will conduct a simple FMECA to be able to inductively understand the failure modes and preventive maintenance
actions to prevent occurrence.
e) I will ensure that strategies are created on the CMMS with proper call dates and generation of work order call object.
