INTRODUCTION

Objective structured clinical examination (OSCE) is a form of


performance-based testing used to measure candidates clinical
competence.
During an OSCE, candidates are observed and evaluated as they go
through a series of stations in which they interview, examine and treat
standardized patients (SP) who present with some type of medical
problem.

OCSE is an assessment tool in which the components of clinical competence such as
history taking, physical examination, simple procedures, interpretation of lab results,
patient management problems, communication, attitude etc. are tested using agreed
check list and rotating the student round a number of stations some of which have
observers with checklist. (According to Ananthakrishnan)
OSPE
The objective structured practical examination is a new pattern of practical
examination. In OSPE each components of clinical competence is tested uniformly
and objectively for all the students who are taking up a practical examination at a
given place.
PURPOSE OF OSCE/OSPE

1. Measures clinical skills


2. Promote structured interaction between student and examiner.
3. Make structured marking scheme possible.
ORGANIZING THE OSCE/OSPE
1. The OSCE examination consists of about 10-15 stations, each of which required 4-5
minutes.
2. All station should be capable of being completed in the time.
3. The students are rotated through all stations and have to move to the next station at
the signal.
4. As the stations are generally independent students can start at any procedure stations
and complete the cycle.
5. Thus, using 15 stations of 4 minute each, 15 students can complete the examination
within 1hour.
ADVANTAGES OF OSCE/OSPE

1. More valid than the traditional approach to clinical examination.


2. Can be used in large number of students.
3. Examiners can decide in advance what is to be tasted and can then design the
examination to test these competencies.
4. Better control on the content and complexities.
5. More reliable.
6. More practical because it can be used with a large number of students.
DISADVANTAGES OF OSCE/OSPE

1. Demanding for both examiner and patient.


2. Time consuming, careful preparation is essential.
3. Need of structured checklist.
4. Maintaining uniform difficulty level is not always possible .
