Ownership/Staffing

High Point Sports Rehabilitation and Physical Therapy LLC will be owned and operated by Jacob
Shuler, Cooper Alspaugh, Cole Simmons, Drake Synder, Zach Lamphere, and Tyler Graft. Each
owner will share 1/6 of the operation responsibilities and profits.

Costs
• Equipment and renovations total= $40,000
• First year rent will total at $72,000 ($6,000/month)
• Utilites and other monthly operating costs approximately $1000
• Combined owner contributions - $20,000
• Salaries= Owners will not make a profit unit the business breaks even on
initial $40,000 investment and monthly operating costs.
Break Even at one year (worst case scenario) 
Finacial Goal- The goal of our revenue expansion is to have 150 concierge members by the end of our first year.
This would bring in $50,000 per month in memberships.
