The incidence of women delaying their first
pregnancy until their late 30s or early 40s is
increasing. 12% of births in the United States
today are to women over age 35; 2% to 4% are to
women over age 40 (NCHS, 2009).
A woman over age 40 is more
likely than a younger woman
to have a previously diagnosed
condition such as
hypertension, varicosities, or
hemorrhoids.
By age 40, a woman
usually has a major role
change to undertake
during pregnancy.

Woman in this age group who is pregnant
may begin to feel ambivalent during
pregnancy.

Help her balance her life to manage

Determine if childbearing is timed correctly.

If not, they may need more time to accept the


pregnancy and to becoming parents.
Women who have a child at
age 40 fall into one of two
groups
Those who are having
their final child
Those who have
delayed childbearing
because of education or
career and are having
their first child.

Health History

Determine if she has been taking self-medication for reported symptoms


and if she has been drinking heavily.

Woman’s family profile: if there has been major life changes, her source
of income, if there are people dependent on her, any family history of
diseases.

Ask her about her daily routine and her personal habits.
Physical Examination

NUTRITION
Assess number of meals and adjust
her nutritional intake.
Urge her to subtitute a caffeine-free
soft drink; subtitute milk or juice or
decaffeinated coffee.

Recommend milk and calcium rich


foods such as puddings and yogurts.
PRENATAL CLASSES

Exercise classes
During pregnancy provide women with an
oppurtunity to interact with other
pregnanct women while benefiting from a
carefully monitored workout.

COMPLICATIONS OF

Hypertension of
pregnancy
More likely to occur in
nulliparous and those with
already elevated blood
pressure.

Have a rest for a good


portion of each day to
reduce hypertension.
Preterm or Post Term
Birth
Pregnant mothers at 40 are more at
risk for preterm birth.

Post term birth can occur due to that


cervical dilation is not as
spontaneous for older women.
Caesarean Birth
Many pregnant over 40 resort
to CS birth to reduce risks for
both mom and baby.

Failure to progress in
labor
Cervical dilatation may not occur as
spontaneously in older women.

Labor may be overly prolonged and can


place risk for baby.
Postpartal Hemorrhage
Uterus may not contract as readily during postpartum due
to inelasticity. Thus, older women have higher risk for
postpartum hemorrhage and perineal-anal tears.

Give close observation and assess the amount of her


lochial flow.
Difficulty Accepting the Event
They may have second thoughts about the
baby especially during intrapartum and
postpartum periods.

Wishes to be more informed and confident


in this phase of life as they are in other areas.

postpartum rest.
