Nursing care to Mr. S.T.

With
Congestive Heart Failure in
Bougenville Ward, Liunkendage
Hospital, Tahuna

Based on the nursing care to the
client Mr. S.T. With Congestive
Heart Failure in Bougenville
Ward, Liunkendage Hospital,
Tahuna in 3 days, started on
March 11- March 13, 2015.
The application of Nursing
Care had done in 5 steps,
consist of assesstment,
nursing diagnoses,
intervention,
implementation, and
evaluation.
Assessment
 Focus data on the assessment
found on the client with Congesti
Heart Failure as follows:
shortness of breathing, coughing,
weakness, loss of appetite, nausea
/ vomiting, oedema, decreased of
self-concept
 On physical examination, auscultation of
the thoracic region there were weezhing
and Ronchi, the presence of extra heart
sound (S3 and S4). On ECG result there
were atrial or ventricular hypertrophy,
and ischemia. Thoracic images revealed a
picture of an enlarged heart. Oximetry
showed low oxygen saturation.
 On the other hand, client was admitted to
the hospital because of dyspnea or
diffulties of breathing. When assed client
said that breathlessness since one week
ago and oxygen via nasal canule installed 2
- 4 liters / min and IVFD NaCl 0.9% to 8
GTT / min
 Client complained shortness of breating
even while resting so that the client need
help from families and caregivers in his
activities. Client also experienced
weakness, and pale. Client’s vital signs
results were BP = 130/70 mmHg, p = 98
times / min, R = 28 times / minute, T = 37
℃.
Nursing diagnosies
 Nursing diagnosis is to identify, focusing,
and addressing the specific needs of the
client as well as the response to actual
problem. From the literature learned by
the author, there 6 diagnosis for the client
with Congestive Heart Failure.
 While the review of this case found 2
diagnosis
Intervention
Nursing intervention is the formulation of
goals, actions, and assessment of the
Nursing care to the client based on the
analysis of assessment and health
problem. In this section, the author does
not write all the interventions applied to
the client. It is adapted to the conditions
and needs of the client.
Implementation
 Implementation is the application of the
nursing care plan, there are some
interventions that can not be
implemented by the author because there
are many inhibiting factors such as limited
facilities or tools and also there are the
rules stated that student does not allow
to handle some procedure.
Evaluation
 The evaluation is conducted to assess
whether the problem can be solved in a
natural client or not, and need to be
assessed, planned. In the evaluation phase
of the data taken in the form of subjective
data and objective data. Based on the
evaluation of the two diagnoses, there is
no diagnosis has resolved. This is due to
the limited time which is too short.
