Carbidopa prevents peripheral breakdown of levodopa, making more levodopa available for transport into brain. Nursing Considerations and Teaching
Side effects: FREQUENT: Uncontrolled movements of face, tongue, arms, upper body; nausea/ vomiting (80%); anorexia (50%). OCCASIONAL: Depression, anxiety, confusion, nervousness, urinary retention, palpitations, dizziness, lightheadedness, decreased appetite, blurred vision, constipation, dry mouth, flushed skin, headache, insomnia, diarrhea, unusual fatigue, darkening of urine and sweat. RARE: Hypertension, ulcer, hemolytic anemia (marked by fatigue). Toxic effects: High incidence of involuntary choreiform, dystonic, dyskinetic movements in those on long-term therapy. Numerous mild to severe CNS and psychiatric disturbances may occur (reduced attention span, anxiety, nightmares, daytime somnolence, euphoria, fatigue, paranoia, psychotic episodes, depression, hallucinations).
Nursing Considerations: Instruct patient to void before medication is given. Be alert to neurologic effects (headache, lethargy, mental confusion, agitation). Monitor for evidence of dyskinesia (difficulty with movement). Assess for clinical reversal of symptoms (improvement of tremor of head and hands at rest, mask-like facial expression, shuffling gait, muscular rigidity). Patient teaching: Avoid tasks that require alertness, motor skills until response to drug is established. Avoid alcoholic beverages during therapy. Sugarless gum, sips of tepid water may relieve dry mouth. Take with food to minimize GI upset. Effects may be delayed from several wks to mos. May cause darkening of urine or sweat (not harmful). Report any uncontrolled movement of face, eyelids, mouth, tongue, arms, hands, legs; mental changes; palpitations; severe or persistent nausea/vomiting; difficulty urinating if taking orally-disintegrating tablet.
