Risk Matrices

(1) Risk probability is defined as the likelihood that an unsafe event or condition might occur. The likelihood or accident frequency resulting from the
identified hazards or safety deficiencies shall be estimated using Table 1 below.

Table 1: Risk Probability

Probability of Occurrence

Quantitative definition Meaning Value

Almost Certain Continual or repeating experience 5

Frequent Common occurrence 4
Occasional Possible or known to occur 3
Remote Not likely to occur under normal circumstances 2
Rare Not expected to occur but still possible 1

(2) Risk severity is defined as the potential consequences of an unsafe event or condition, taking as reference the worst foreseeable situation. The severity of
accidents resulting from the identified hazards or safety deficiencies shall be estimated using the definitions in Table 2 taking into account a pessimistic but
reasonable assessment of their consequences.

Table 2: Risk Severity

Severity of Occurrence
Aviation definition Meaning Value

Catastrophic  Fatality, fatal diseases or multiple major injuries 5

 Serious injuries or life-threatening occupational disease (including amputations, major fractures, multiple injuries, 4
Major
occupational cancer, acute poisoning).

 Injury requiring medical treatment or ill-health leading to disability (includes lacerations, burns, sprains, minor 3
Moderate
fractures, dermatitis, deafness, work related upper limb disorders).

 Injury or ill-health requiring first-aid only (includes minor cuts and bruises, irritation, ill-health with temporary 2
Minor
discomfort).

1
Negligible  Not likely to cause injury or ill-health.

(3) The risk category of the identified hazard or safety deficiency shall be assessed using Table 3 below by cross-referencing estimated risk probability
with the risk severity.

(4) The risk tolerability determined from Table 3 is presented in Table 4 below.

Table 4: Risk Tolerability

Risk Level Acceptability Of Risk Recommended Actions

No additional risk control measures may be needed. However, frequent review may be needed to ensure that the risk level
Low Risk Acceptable
assigned is accurate and does not increase over time.

A careful evaluation of the hazards should be carried out to ensure that the risk level is reduced to as low as is practicable
Medium Risk Moderately Acceptable within a defined time period. Interim risk control measures, such as administrative controls, may be implemented.
Management attention is required.

High Risk High Risk level must be reduced to at least Medium Risk before work commences. There should not be any interim risk
control measures and risk control measures should not be overly dependent on personal protective equipment or
Not Acceptable
appliances. If need be, the hazard should be eliminated before work commences. Immediate management intervention is
High Risk required before work commences.

Risk Control

Hierarchy of Control

The control of hazards and reduction of risks can be accomplished by following the Hierarchy of Control (Elimination, Substitution, Engineering, Administration,
and PPE). These control measures are not usually mutually exclusive. Generally, it may be more effective to use multiple control measures, for example, engineering
controls work better with administrative controls like training and Safe Work Procedures.
