    1. Booking Iterinary Ticket ° Consular Hrs.
    2. Passport (valid for at least six months) Monday-Thursday
    3. 2 pictures (passport size) 9:00am-12nn
    4. Application form (duly filled in)
    5. Personal apperance 5-7 Days Processing
    6. Visa fee *
    7. Photocopy of Passport
    8. Proof of residence (ACR & ICR) (For Non-Filipino Residence)

    2 Photocopy of Passport & NIC of the sponsor 2 Membership Certificate of Chamber of Commerce in Pakistan
    3 NBI Clearance (authenticated by DFA) 3 Original recommendation letter from Chamber of Commerce
    4 Bank certificate of Industry in Pakistan
    5 Employment Certificate 4 Letter of request & Certificate of Employment from the
    6 Confirmed ticket and Hotel applicant's firm.
    5 Company registration of the applicant (SEC)
    6 NBI Clearance (authenticated by DFA) Filipino National
    7 Bank certificate
    NOTE: Kindly attach copies of Pakistan visa for those who already traveled to Pakistan.

    1 Approval from the Ministry of Interior 1 Computer proforma (duly filled in)
    2 Job Contract (attested by the Philippine 2 Letter from Missionary organization certifying that the applicant
    Embassy Islamabad) duly signed by is a bonafide member of that society/organization concerned
    the employee and the employer in the presence and that society etc. takes full responsibility for his/her
    of two witnesses maintanance/boarding/lodging & retrun journey in case
    3 NOC from the Philippine Embassy Pakistan repatriation from Pakistan becomes necessary
    4 Acknowledgement of the Contract Declaration to abide by and respect the laws of Pakistan
    5 Computer proforma (duly filled in) and stay away from internal politics or religious acrimony etc.
    6 Medical fitness certificate 3 Bio-data for the last 10 years
    7 Original and photocopy of all documents Copies of educational degrees/diploma/certificate with english
    8 NBI Clearance (authenticated by DFA) 4 translation attested by the DFA.
    9 Copy of NIC of the employer 5 Letter from the concerned institution in Pakistan inviting
    (in case of Pakistani citizen) the Missionary worker
    10 Passport photocopy 6 In case of replacement:
    (in case of foreign employer) Complete details of the person being replaced showing
    the exact date of final exit & visa position at that time
    alongwith photocopies of passport pages indicating final exit,
    final visa as well as the cancellation of remaining visa, if any.

    ° in case of work / housemaid visa - one way ticket is also acceptable
