Customer Declarations:
I/we certify that the information provided by me in this application is true and correct in all respects and The Hongkong and Shanghai
Banking Corporation Limited, India (HSBC India) is entitled to verify this directly or through any third-party agent. I/we also agree that if
any of the above details or the confirmation made by me/us is found to be incorrect by HSBC India is not bound to pay any interest on
the Savings Account(s) opened by me/us and is entitled to restrict operations in the account or cancel the credit card limit and also to
terminate the account relationships. The HSBC representative has provided me with the necessary information to make an independent
decision to apply for the opening of Savings Bank Account (Bank Account) and Credit Card with the Bank. I/we understand the features
of the products, its terms, conditions, and applicable charges on availing the services/product from HSBC.
I/we have also submitted my personal and other relevant details, including KYC documents as required, to open my accounts with
HSBC. Based on the information provided to the HSBC bank representative verbally and in writing, based on the information provided to
the HSBC bank representative, I/we understand that I/we will subsequently receive an electronic version of the filled-up account opening
form from the bank on the e-mail ID provided by me and mentioned hereinabove. I/we confirm that by providing my electronic consent,
I/we will deem to have confirmed the factual correctness of the contents of the electronic version of the account opening form for
savings bank account and credit card and I/we will abide by the declarations as mentioned in the said electronic form.
I/we confirm that I/we are providing my digital consent to the electronic version of the account opening form that will be sent to me
subsequently and I/we also confirm having read and understood the account rules/terms and conditions governing the accounts, Credit
Cards and the various services including not limited to Debit Card/ATM/Mobile alerts in respect of the account(s), or changes thereof and
any further as amended from time to time amendments made by HSBC India as made from time to time. (Account rules/terms and
conditions is available on www.hsbc.co.in. By signing this form, I/we am/are agreeing to the terms and conditions applicable on the
opening of the Bank Account with HSBC India.
I/we agree and accept that HSBC India may need to share or transfer data or information about me/us to any third-party service provider,
whether located overseas or in India, who provides outsourced services to HSBC India in connection with the operation of HSBC India
business, in accordance with the applicable guidelines of the Reserve Bank of India. I/we understand that the details as appearing on the
Proof of Identity/Proof of Address document submitted my/us will be captured in the records of the Bank and, I/we shall not raise any

If nomination being done: I/we confirm to have given my/our digital consent to the nomination form and confirm the factual correct-
ness of the contents of the electronic version of the nomination form.
If applied for Credit Card: I agree that credit limits are subject to periodic review and will vary based on the bank’s credit assess-
ment of my card account, expressly authorise HSBC India to decrease the credit limit on my credit card, based on its credit assess-
ment, without prior notice to me.
For cases with PAN Card: I/we authorise HSBC to validate my/our Permanent Account Number (PAN) from the database authorized
by Income Tax Department.
For Signature difference (if any): I/we understand that my/our signature on the Account opening form (for savings bank account
and credit card) and nomination form on the digital platform may differ with the signature on HSBC India’s record or on the
identification document submitted by me whilst applying for the account opening. In such a scenario, I/we shall not raise any dispute
in relation to the same. In case of any discrepancy in the signature, the signature in this form shall prevail. Based on the above
representation I/we request the Bank to process this application.
This is to confirm that I am voluntarily providing a physical copy of my Aadhaar card/e-Aadhaar card in a masked form to HSBC
for the purpose of establishing my/our identity/address proof to set up a banking relationship and process instructions in relation to
the same. I hereby voluntarily keyed in my Aadhaar number towards e-KYC and hereby provide my consent for verification of my
Aadhaar to establish its genuineness through such means acceptable as per UIDAI or under any Act or law from time to time. The
consent and purpose of collecting Aadhaar have been explained to me and I confirm having understood the same. I acknowledge
that the Bank will store the said Aadhaar number in a masked form in their records and use the same for such purposes as may be
permitted. I also acknowledge that the Bank has provided me/us with alternate options in terms of identity documents that could be
provided in lieu of Aadhaar for setting up a banking relationship.

Staff Declaration: I have verified the name of the customer on the Account Opening Form (AOF) for Savings
and Credit Card with the KYC documents provided and confirm that it meets the standards of CDD. I also hereby
confirm that customer’s photo on PI and PA (if photo id) matches with the photo on the AOF.
