qualify for benefits, based on average hours worked, enrollment is time sensitive. We encourage you to understand your

information about our benefit programs. After one year of employment, we will verify your actual weekly hours and
adjust your status (full time or part time) at that time. Your average hours paid will be verified annually.

There are several important forms you will need to complete on your first day of work, including the Form I-9 for
verifying your employment eligibility. Please bring your work authorization documentation with you on your first day of
work (see what documentation to bring in on page 4 at http://www.uscis.gov/files/form/i-9.pdf). Failure to provide
acceptable documentation within 3 business days following your first day will result in automatic unpaid suspension for
48 hours and subsequent termination of employment. The Company also utilizes E-Verify to ensure a fully authorized
workforce. Additionally, please bring a voided check or the routing number from your bank account to enroll in direct
deposit. If you do not have a bank account, you will be set up with our payroll card program.
