During the COVID-19 pandemic and as we prepare to return to work, Monster Energy is closely monitoring
guidance from state and local governments as well as the CDC on travel and return to work protocol.
Monster Energy cares about your health and safety and wants to provide you with the following important
information.
The CDC provides the following guidance on how to prevent the spread of COVID-19:
https://www.cdc.gov/coronavirus/2019-ncov/prevent-getting-sick/prevention.html. The CDC’s current
data suggests that the individuals most likely to develop severe forms of COVID-19 are those who are 65
years of age or older, people of all ages with underlying medical conditions, particularly if not well
controlled, including: chronic lung disease or moderate to severe asthma, serious heart conditions,
immunocompromised, severe obesity (body mass index [BMI] of 40 or higher), diabetes, chronic kidney
disease undergoing dialysis, or liver disease. Please refer to the website for the current list:
https://www.cdc.gov/coronavirus/2019-ncov/need-extra-precautions/people-at-higher-risk.html. If you
fall into any of these high-risk categories and do not wish to travel as required by your position or you have
any concerns about your ability to perform your job duties due to the travel requirements of you position
as a result of COVID-19, please contact [email protected] immediately. If you do not notify
[email protected], Monster Energy will assume that you have made the decision to proceed
with your travel requirements and job duties with full knowledge of the risks associated with COVID-19.
You are aware of all applicable state and local stay-at-home orders, you agree to comply with all
requirements of the applicable orders, and you agree not to violate any of the applicable orders during
travel. You acknowledge and understand that, due to the COVID-19 pandemic, there are increased risks
associated with travel and you are voluntarily making the decision to travel as required by your normal job
duties. The current risks and guidance from the CDC related to domestic travel is provided within the links
provided in this Notice and Acknowledgment. You agree to read and adhere to all of the applicable orders
in any jurisdiction where you are traveling. You acknowledge and understand that you may be subject to
quarantine when you arrive at a destination and/or return home after traveling depending on the location to
which you traveled. For up to date information on the CDC’s travel guidance during the COVID-19
pandemic, please see the CDC website at: https://www.cdc.gov/coronavirus/2019-
ncov/travelers/index.html. You acknowledge that you are voluntarily making the decision to travel in
connection with your job duties and that this decision involves an inherent risk of contracting COVID-19,
personal injury and even death. You understand and acknowledge that Monster Energy has advised that
while traveling you should comply with all applicable CDC guidelines as well as all state and local
government stay-at-home orders. You hereby acknowledge that you know of no medical reason why you
should not travel.
YOU ACKNOWLEDGE THAT YOU HAVE READ AND UNDERSTAND THE FOREGOING
