    Management and hired the services of vehicle registration number ________________

    4. That I also have got the antecedents of the owner/ driver verified from the police

    5. That I have also cross checked and ensured that owner/ driver of the vehicle having
    required permit prescribed by the rules of the transport department concerned for
    ‘Passenger Transport Vehicle’.

    6. That I have gone through the school cab policy / guidelines and directions issued by
    the transport department.

    7. That the vehicle I am engaging complies with the guidelines and the rules of transport
    department applicable for the school cabs. I/We further understand and agree that our
    ward is availing the same solely at my /our decision.

    8. I acknowledge that I have independently made the vehicle arrangement for my ward,
    understanding that the school management / authorities are not liable in any way for
    these arrangements.

    9. I declared that I have submitted an attested copy of the following documents with the
    School Authorities.

    a. Cab/Van/Bus driver’s driving license


    b. Aadhar card of Cab / Van / Bus driver
    c. Registration Book & Permit of the vehicle
    d. Police verification of Cab / Van / Bus driver
    e. Psychometric test documents of the Cab / Van / Bus driver
