2) BI Publisher Desktop 12.2.1.4 (32 bit) is installed without any error.

SOLUTION

While the root cause of the issue is not determined, it is made Active with the steps below.

1) On the Add-ins, set "Manage" on the bottom to "Disabled Items" then click Go.
2) BI Publisher Template Builder for Word is moved to Inactive Application Add-ins.

3) Set "Manage" on the bottom to "COM Add-ins" then click Go.
4) BI Publisher Template Builder for Word is displayed. Click the checkbox then OK.
5) BI Publisher Template Builder for Word is Active and the menu appears on Word ribbon.
