Greetings!
We are Reymark A. Catalla, Malie T. Cotejar, Kristine Camille P. Darole, and China Marie S. Pico,
fourth-year college students, taking up the Bachelor of Secondary Education Major in Mathematics. As
one of the requirements in our teaching internship, we are currently working on our action research
entitled “Investigating the Effectiveness of Game-Based Mathematics Activities In Enhancing
Student Performance.” We are in the process of data gathering. On this basis, we are seeking your
approval to conduct our action research and implementation.
The main goal of this action research is to investigate the effectiveness of game-based mathematics
activities in enhancing Grade 9 students' performance at Alangalang Agro-Industrial School.
Rest assured that the data to be gathered from the students will be used only for research purposes and
will be treated according to the Data Privacy Act of 2012.
Attached herewith is the research instrument for your guidance. We are looking forward to a favourable

note, we are communicating with you to seek permission to conduct a pilot test of the research
instrument among 30 Grade 9 students in your school.
The goal of this study is to analyze the impact of game-based design in mathematics activities on
learning outcomes of Grade 9 students in Alangalang Agro-Industrial School.
Rest assured that the data to be gathered from the students will be used only for the research purposes
and will be treated according to the Data Privacy Act of 2012.
Attached is the research instrument being submitted for validation. The statement of the problem is also
attached for the perusal.
