Medication Calculation Review

Metric System:

 Mass

÷1000 ÷1000
Microgram Milligram Gram

Example: 100 mg = 0.1 gram

X 1000 X 1000
Gram Milligram Microgram

Example: 1 gram = 1000 mg = 1000000 mcg

 Volume:

÷1000 ÷1000
Micro-Liter Milliliter Liter

Example: 100 ml = 0.1 liter

x1000 x1000
Liter Milliliter Micro-Liter

Example: 1 liter = 1000 ml = 1000 cc

Milliliter (ml) and cubic centimeter (cc) are the same and used interchangeably

Law of Definite Proportions

 By the Law of Definite Proportions, IF

A C
____ ____
=
B D

Then,

AxD = BxC

BxC AxD
A = __________ B = _________

D C

AxD BxC

C = _________ D = __________
B A

Required Dose
____________________________ Volume
X
Available Dose

Example: Physician ordered 12.5 mg of Captopril (Capoten) to be administered

to a patient. The nurse found that the pharmacy supplied Captopril as 50 mg
tablets. How many tablets should the patient get?

Required Dose = 12.5 mg

_____________________________ X Volume = 1 tablet = 0.25 tablet (Quarter)
Available Dose = 50 mg

Modified Jan 2022

 The other way to solve this exercise is:

1 tablet of Captopril 50 mg

? tablet of Captopril 12.5 mg

12.5 mg X 1 tablet
? = _____________________________ = 0.25 tablet

50 mg

If you could notice, 12.5 mg is the required (ordered) dose; 50 mg is the

available (stock) dose. For managing tablets, the volume will be
considered as (1) quantity.

Example: The physician ordered morphine sulfate elixir to be given to the patient
as 20 mg oral every 6 hours. The bottle of morphine reads as: Morphine Sulfate 5
mg in 2 mls. What is the amount to be given to the patient?

Required Dose = 20 mg
_____________________________ X Volume = 2 ml = 8 ml
Available Dose = 5 mg

If you follow the Law of Definite Proportions:

2 ml
5 mg of Morphine

20 mg of Morphine ? ml

20 mg X 2 ml
? = _____________________________ = 8 ml
5 mg

Fluid Rate:

(VOLUME) (ml) X (DROP FACTOR) (drops per ml)

RATE (drops per minute) = _______________________________________________

TIME (minutes)

 REMEMBER TO CONVERT TIME TO MINUTES.

 REMEMBER TO CONVERT VOLUME TO ML (SAME AS CC).
 DROP FACTOR DEPENDS ON THE GIVING SET (TUBING); HOW MANY DROPS
TO FORM ONE ML.

Rate, Time Volume, Drop Factor

(VOLUME) (ml) X (DROP FACTOR) (drops per ml)

RATE (drops per minute) = _______________________________________________

TIME (minutes)

RATE (drops per minute) X TIME (minutes)

(VOLUME) (ml) = _______________________________________________

DROP FACTOR (Drops per ml)

RATE (drops per minute) X TIME (minutes)

DROP FACTOR (Drops per ml) = _______________________________________________

(VOLUME) (ml)

(VOLUME) (ml) X (DROP FACTOR) (drops per ml)

TIME (minutes) = _______________________________________________

Modified Jan 2022

RATE (drops per
Fluid Rate minute)

Example:
The physician ordered fluids of one liter of D/W 5% to run over 8 hours. The tubing set
delivers 15 drops per ml. What is the fluid rate?

(VOLUME) (ml) X (DROP FACTOR) (drops/ml)

RATE (drops/minute) = _______________________________________________

TIME (minutes)

1000 (ml) X 15 (drops/ml)

(drops/ml)
RATE (drops/minute) = __________________________ = 31 (drops/minute)
8 X 60 (minutes)

 WHEN DROP FACTOR IS 60 DROPS/ML, THE RATE IN DROPS/MINUTE IS THE SAME

RATE AS ML/HOUR

Drug Concentration

Drug Concentration refers to the amount of medication that is present is one ml of

volume.

Example: Heparin 25,000 Units is added to 250 ml of D/W 5%. What is the concentration
of Heparin in the bag?

Total Dose in the Bag

Concentration = ___________________________

Total Volume of the Bag

(drops/ml)

RATE (ml per hour) = ___________________________________________

CONCENTRATION (gram, mg, unit per ml)

DOSE (gram, mg, unit) = RATE (ml/hour)

X CONCENTRATION (gram, mg, unit/ml)

Example: A patient is receiving 1000 Units of Heparin per hour intravenously. The
pharmacy delivers a bag of 500 ml of NS containing 20,000 Units of Heparin.
– What is the drug concentration?
– What is the rate of infusion in ml per hour?

TOTAL DOSE 20000 Unit

CONCENTRATION = =
TOTAL VOLUME 500 ml

DOES 1000 Unit/Hour

RATE = ____________________ = ___________________ 25 ml/Hour

 The other way to solve this exercise is:

Example: A patient’s IV fluid is ordered as: D/W 5% + 80 mmol of Potassium
Chloride per liter, running @ 100 ml per hour. How many mmol of KCl the patient
is receiving in an hour?

DOSE (gram, mg, unit) = RATE (ml/hour)

X CONCENTRATION (gram, mg, unit/ml)
