Budget Proposal
1. This is a general budget proposal form. Erase the section that is not
needed in your study (e.g., if life science, do not include software and
licensing).
2. In section A (personnel), include the sponsor (consultant or scientist that
will guide you during your study. Include if it is free. If not, please it should
not cost much (huwag magpapataga).
3. Section B (materials and section and procedures), if you must buy the
materials, include it. If it is included in the laboratory procedure, just
include the procedure and how much it costs. Please take note that you
need to include the right price from the institution and not just what your
consultant told you.
4. Section C (travel expenses and printing), assume the price of going back
and forth to laboratory for plant verification, testing/experimentation, etc.
Printing of manuscript should also be included.
5. Your study must not cost much more than 5,000 pesos, or must not exceed
10,000 pesos max. If it is inevitable to exceed the price if you are going to
use cell lines, include the right amount from the laboratory or institution.
6. Have your budget proposal form signed by your parents. If parent is not
present, have someone (guardian) sign it in place of the parent. Make
sure that your parent must be informed as well.
NOTE: Do not include this page and the budget overview when you pass this
form. Just the budget form and the parental consent. Use this form when your
teacher checked your previous budget proposal.

PARENTAL CONSENT
I, as a parent of one of the researchers in this study, hereby give my consent and support for
my child's participation in experimental research. I am aware of the potential costs and risks involved
in this research, and I fully support my child's pursuit of knowledge and scientific exploration.
1. I am the legal parent/guardian of one of the researchers in this study and I have the authority
to provide consent for their participation in experimental research.
2. I have discussed the nature of the research project with my child and understand its
objectives, methodologies, and potential risks involved.
3. including but not limited to materials, equipment, and possibly professional guidance.
4. I am willing to provide financial support to the best of my ability to facilitate my child's research
endeavors.
5. I understand that despite precautions taken, there may be inherent risks associated with
experimental research, including but not limited to physical injury, property damage, or
unforeseen consequences.
6. I agree to release and hold harmless any individuals or entities associated with the research
project, including but not limited to educational institutions, mentors, advisors, and sponsors,
from any liability arising from my child's participation in the research.
7. I recognize the importance of fostering my child's curiosity, critical thinking skills, and passion for
scientific inquiry, and I fully support their intellectual and academic pursuits.
I acknowledge that my child's participation in experimental research is voluntary, and they may
choose to withdraw at any time without prejudice.

- Laboratory technicians: The one who will execute the process or guide you in conducting the
services inside the laboratory (if your consultant is not the one who would do it with you in
laboratory).
- Consultant: Budget allocation for specialized consultants/experts in life science and other
biological science procedures.
-
B. Equipment and Supplies and Procedures:
- Core Facility Usage: Includes high-performance centrifuges, microscopes, (fluorescence,
electron), PCR machines, incubators, shakers, and other specialized equipment (if the
payment for these materials is separate from the services offered by a laboratory)
- Consumables: Includes reagents, cell culture media, Laboratory glassware and plasticware (if
payment for these materials is separate from the services offered by a laboratory)
- Biological Samples: Procurement of specific biological samples like cell line, bacteria, etc.
- Laboratory procedures and services: This includes the services or procedures that a laboratory
offers intended for your study. Ex. Assays, cell culture, genomics, bioinformatics, identification
and verification of plants used, etc.)
E. Contingency Fund
- Unforeseen Expenses: A percentage (5% or 10% of the total budget) of the budget allocated
as a contingency for unexpected costs or additional requirements that may arise during the
project.
