    Hello! We are Grade 12-Orion students from Nueva Estrella Santiago Dulos Academy, Inc. Currently, we
    are conducting our research entitled “THE IMPACT OF ONLINE GAMES TO THE ACADEMIC
    PERFORMANCE OF THE STUDENTS IN NUEVA ESTRELLA SANTIAGO DULOS ACADEMY, INC.”
    The main objective of this research is to determine the impact of playing online games to students’
    academic performance in Nueva Estrella Santiago Dulos Academy, Inc. This survey will just take 5-10
    minutes of your time. Rest assured that your personal data will be confidentially protected. There will be no
    wrong answer in this survey. Your answers will be a great help for the success of this study. Thank you so
    much!

    The survey questionnaire is composed of four (4) parts. The first part is to determine the socio-
    demographic profile of the respondents such as age, gender, ethnicity, and address. Then, the second part
    covers the student’s daily session duration for playing online games. Examples of online games that the
    students usually play will be the main topic for the third section. In the last part, it focuses on the impact of
    playing online games to the academic performance of the students in Nueva Estrella Santiago Dulos
    Academy, Inc.
