STEPS FOR EXAMINATION Of INGUINAL HERNIA

1- Greeting, Introduction, Consent


Asstam-0-Alaikum, my name is ……….., I am going to examine you, you won’t feel any pain.
What is your name and age?
What is your profession?

2- Screen/Privacy/ Chaperon

3- Exposure
Epigastrium should be visible.
Ask the patient to stand straight, take off his shalwvar and fold the shirt up.

4- Look
Observe from front and right side.
Whether the swelling is Inguinal / Inguinoscrotal or Pure scrotal
Recurrent swelling, look for scars
Ask for cough and look for a visible cough impulse.
Ask for constipation and any urinary complaints.
Site, Size, Shape, Overlying Skin, Scars, visible pulsations, distended veins over the abdomen
Penis - Circumcision, Chordae
Scrotum - Varicocele
Ask the patient to cough and look at the other hernial orifices (Paraumbilical, umbilical, Epigastric,
lumbar and incisional hernias).

5- Palpation
Warm your hands.
Ask for any tenderness.
Check temperature with the back of your hand
Check for tenderness over the swelling
Ask the palient to cough and feel for cough impulse over the swelling
Palpate scrotum, testes, vas deferens
See undersurface of scrotum for fungal infection, sebaceous cyst
Look at penis for meatus, stricture, circumcision
Try to get above the swelling (Can not get above the swelling means it is an inguinoscrotal hernia),
Can get above the swelling means it is a pure scrotal swelling most likely hydrocele)
Ask the patient to reduce the hernia, if he cannot reduce don’t try to reduce
Perform deep ring occlusion test (1.25cm above the middle inguinal point), use tip of right thumb for
right sided hernia and left thumb for left sided hernia.
Check Inguinal lymph nodes
Palpate abdomen deeply, see ascites by shifting dullness and fluid thrill
Palpate all other hernial orifices
Percuss urinary bladder
Check spine tenderness
Perform a PR examination - for prostate, hard faecal matter
Auscultate over the swelling for bowel sounds
