    2. Certificate of General Point Average (if GPA is not shown in the TOR)
    3. PSA Birth Certificate (for non-USM graduates)
    4. Certificate of Transfer or Honorable Dismissal (for non-USM graduates)
    5. Certificate of Good Moral Character (for non-USM graduates)
    6. PSA Marriage Certificate (for married applicants only)
    7. GLT Result

    I also hereby waive my right from claiming any credit of grades gained if these
    documents are not submitted or if there is discrepancy between original and online
    submitted documents.

    I also agree that upon taking GLT and my score falls below the cut-off score, then
    I need to take additional courses of at most six units to enhance my competencies, and
    that these units will be part of my requirement for graduation.

    With the understanding of the above conditions, I hereby express my intention to


    enroll at the USM Graduate School for the SY 2022-2023 and submit all original
    documents to the registrar’s office before the end of my 1st Semester SY 2022-2023.
