Salary Scale - £40,995 - £64,461 per annum (pro rata if part time)

Applications are invited for the position of Clinical Development Fellow (CDF) in Acute Internal
Medicine. These posts comprise a clinical role working in the Acute Medicine Department at
Forth Valley Royal Hospital in conjunction with one day per week of professional and
departmental development time.

This would suit candidates who have recently completed or about to finish either UK Foundation
Programmes or Core Medical Training and have been assessed as being competent in
delivering acute medical care under supervision. The post will give successful applicants the
opportunity to gain experience in Acute Medicine before committing to CMT, ACCS or indeed
higher specialty training in Acute Medicine, or would provide a professional development adjunct
if interested in another non-acute specialty longer term.

1. JOB DETAILS

Term: One year fixed term from Aug 2024

Time Commitment: Full Time

Remuneration: As per national T&Cs (grade specific)

2. JOB PURPOSE

The purpose of this role is two-fold: firstly, to develop clinical experience and competence
in Acute Medicine through the delivery of clinical care; and secondly to provide
opportunity for purposeful non-clinical personal and professional development to assist
CV advancement and departmental service improvement.

Clinical role

The clinical aspect of this role will be based in the Acute Medicine Unit at Forth Valley
Royal Hospital. This unit comprises the following:
 Combined Assessment Unit (16 beds and chaired area)
 Acute Assessment Unit (54 beds)
 Rapid Access Care Unit (RACU) within Ambulatory Emergency Care service

The unit manages patients referred from both Primary Care and the Emergency
Department. There is a dedicated Acute Medicine physician from Monday to Friday and
on-call consultants in General Medicine and Geriatric Medicine daily. In addition to the
team of trainee medical staff, there is an established advanced nurse practitioner service.

The post holder will work under the direct supervision of a Consultant in Acute or General
Medicine. Clinical activities will include the assessment of undifferentiated acute medical
presentations and contribution to wards rounds of admitted patients in the unit.

Depending on previous experience there will be the opportunity to undertake and teach
practical procedures including (but not limited to) lumbar punctures, ascitic taps and
drains, and pleural taps and drains.

There will be an opportunity to contribute to RACU with Consultant Acute Physicians and
advanced nurse practitioners.

There may be an opportunity to gain additional experience in another speciality of choice


on negotiation with base team and destination team.

Non-clinical development role

The non-clinical component of the role is aimed at both professional development of the
post holder and service development within the Acute Medicine department. The details
of the activities this includes will be negotiated on commencement of the post. There is
opportunity to work in an area of mutual professional and departmental interest including
clinical teaching, quality improvement and safety, simulation, management and
leadership development or informatics.

There is an excellent acute medicine teaching program and there would be opportunities
for the CDF to become involved in this, including taking a leadership role if desired. The
successful candidate will have the opportunity to engage in unit quality improvement
including early management of sepsis and the deteriorating patient. Furthermore, there
are opportunities to become involved with clinical audit and the unit has had numerous
successful poster presentations at national meetings. The unit has strong links with the
Society of Acute Medicine (SAM) audit and research group, and the successful candidate
will have scope to participate in National research projects related to Acute Medicine.

3. DUTIES OF THE POST

The exact format of each post will be agreed with the successful applicant, the Clinical
Director of Acute Medical Services, the Clinical Lead and the Director of Medical
Education NHS Forth Valley. However the following general principles will apply. Within
a 10 PAs (programmed activity) contract, each fellow will contribute approximately:

 8 hours per week on average of protected development time


 Supervised in-hours clinical activity in the Acute Medicine Unit
 On call activity including out of hours.

There will be expectation to cross cover colleague’s short notice absences (eg:sickness
absence) as is the case with the remaining trainee workforce.

4. DIMENSIONS

NHS Forth Valley is an integrated NHS Board in Scotland providing primary, community,
mental health and hospital services to a population of approximately 310,000. Currently,
services are provided in one acute hospital, four community hospitals and fifty-six health
centres.

NHS Forth Valley will support the aims to develop and implement the educational
strategy through provision of clinical placements by:

 Oversight of the quality of both post-graduate and undergraduate medical


education in clinical areas of NHS Forth Valley

 Liaison with Director of Medical Education to ensure GMC quality standards are
monitored and reported upon
 Allocating and / or coordinating financial, logistical and event-based resources to
support undergraduate and post-graduate learning
 Integration of training into the demands of a clinical service and ensure safety of
our patients through appropriate and graded supervision and support for training
doctors
 Ensure undergraduate students of medicine are prepared for clinical practice
through engagement with relevant and rewarding experiences in the clinical
setting.

Appointment: the appointment will be on a fulltime basis for one year and subject to
satisfactory on-going appraisal within role.

Salary: In line with current seniority at time of appointment relating to the pay and
conditions of service remuneration of hospital medical and dental staff [NHS Circular:
PCS(DD)2023/01]

5. COMMUNICATIONS AND WORKING RELATIONSHIPS

The post holders will be expected to establish and maintain extremely good communications
and working relationships with a wide range of staff, including:

 Associate Medical Director who has delegated immediate line manager responsibility
to the Acute Medicine consultant body
 Supervising Staff members from Medical, Clinical and AHPs backgrounds
 Academic mentor / Named clinical supervisor
 Director of Medical Education
 Colleagues in training grades at Foundation, Core and Specialty level
 Colleagues from across the multidisciplinary team working in the acute medicine
environment

6. LOGISTICS

Base
The post holder will be based in Forth Valley Royal Hospital.

Annual Leave
Annual leave is in accordance with the nationally agreed terms and conditions of the
grade of applicant.

Medical Clearance
The offer of appointment is subject to the post holder undergoing relevant clearances
and health checks as dictated by HR Medical Workforce Team.

Qualifications and Experience


The post holder must (at the time of commencement of employment) have full
registration with the GMC. Ideally applicants will have recently completed a UK based
Foundation Programme and understand the UK Healthcare system.
