    This Chattel Mortgage made and entered into by and between LENDING CORPORATION, a corporation duly
    organized under the law of the Republic of the Philippines, represented by its President RAFAEL R. DELA CRUZ, of
    legal age, married, Filipino, with principal office address at M. Basa Street, San Pablo City, hereinafter referred to as
    the MORTGAGEE and ____________________________, of legal age, married/single/widow/widower, Filipino, with
    residence and postal address at ___________________________ hereinafter referred to as the MORTGAGOR/S,

    w i t n e s s e t h:

    That the Mortgagor/s does/do hereby convey by way of Chattel Mortgage unto the Mortgagee the following
