SUMMARY
ARTICLE 1:
Patient safety culture assessments allow health care organizations to
obtain a clear view of patient safety aspects , requiring urgent attention ,
identifying the strengths and weakness of their safety cultures help care
giving units identify their existing patients safety problems and evaluate
their scores with other hospitals.
The method was used to identify the articles related to the patient safety
culture is to search in databases (PUBMED,SCOPUS).
Patient safety is main element in health care environment. its properties
organized into seven subcultures ; leadership , teamwork , evidence based
–care , communication , learning , ju8st , patient – centered care.
Patient safety culture is a complex phenomenon . Its assessment required
by international accreditation organizations.

Negative events are increasingly admit as a source of harm to
patients .When such harm occurs problems arise in communicating
the situation to patients and their families. For bring together
reviewed the literature on disclosure across individual and
international boundaries including patients , health care
professionals and other stakeholders .
Research suggests that there is gap between ideal disclosure
practice and reality . Although health care is delivered by
multidisciplinary teams , much focused on physicians experiences.
Disclosure practice can be improved by strengthening policy and
supporting healthcare professionals in disclosing adverse events ,
and by improving provider – patient relationships.

• The Writers describe the small revolution in


patient safety that is taking place in the
basement of their hospital. Bar coding is the
latest technology designed to improve patient
safety at a time when new hospitals are under
increased survey for medical errors .

• Sixty two of the intervention group patients received a total


of 74 suggestions . Among them 63 involved changes in
antibiotic choice , dosing regimen , or route of
administration .
• This is the first randomized study to deter5mine whether
antibiotic choices can be effected in a cost-effective fashion
without sacrificing patient safety . 50% of patients initially
treated with expensive parenteral antibiotics can have their
authority refined after 3 days of therapy and these
modifications result in good clinical outcomes with a
substantial reduction in antibiotic expense.

understanding of medical errors have
expanded rapidly .efforts have focused on
turning evidence into practice ,
diminishing hazards from therapies , and
improving culture and communication.
