British Airways – Young Flyers Aged 14 and 15 Travelling Alone

This form must be completed by the parent/legal guardian (“you”) of the young flyer(s) aged 14 and 15 years old, if travelling
unaccompanied by someone over the age of 16 years old.

Failure by the young flyer(s) to carry this form at all times or present it when requested by British Airways’ personnel or agents
may result in refusal of carriage. The form can be carried in paper or digital format (e.g. a photo or PDF stored on a smart phone
or tablet).

• The minimum age of a person permitted to travel alone on British Airways operated flights is 14 years old in all circumstances.
By signing this form, you give permission for the young flyer(s) listed below to travel unaccompanied by someone over the age
of 16 years old and acknowledge that the young person(s) shall be booked to travel as an adult. BA shall not bear any parental
responsibility for the young flyer(s).

• You understand that adverse weather or other events may occasionally result in disruption entailing an aircraft diversion and/or
overnight stay at, or away from, the scheduled point of departure/destination.

• For young flyer(s) on a flight affected by a disruption event leading to a significant delay, or an overnight stay required at the
scheduled point of departure (either before the outbound or return flight), you agree that you and the relevant person that you
list below can be contacted to arrange the young flyer(s)’s overnight accommodation and personally collect the young flyer(s)
from the airport. If contact cannot be made with you or the listed contact below, you give permission for British Airways or a
representative of British Airways to contact local law enforcement and/or child protection authorities to provide advice and/or
care.

• For young flyer(s) on a flight affected by a disruption event where an aircraft diversion and/or overnight stay is required away
from the scheduled point of departure/destination (including indirect flights connecting though London Heathrow and London
Gatwick Airports), you confirm that you understand that some taxi and hotel providers require formal, written consent for young
flyers(s) to use their services and/or be unaccompanied. By signing this form, you hereby confirm that you give your consent to
the young flyer(s) travelling by taxi and staying in a hotel room unaccompanied.
