1  Unauthorized Work  CTRA shall be reviewed if there is any changes in scope of work, site condition, Issuer
or any new hazard identified during the course of the job. 2 3 L Executor

 Mechanical Failure  Before operating the Manlift, make sure all gates are closed and fastened in their
proper position.
 Keep both feet firmly positioned on the platform floor at all times. Never use
ladders, boxes, steps, planks, or similar items on platform to provide additional
reach.
 Avoid any buildup of debris on the platform floor. Ensure footwear and platform
floor are free from mud, oil, grease, and other slippery substances.
 When two or more persons are in the platform, the operator shall be responsible
for all Manlift operations.

 The Operator shall continuously monitor the situation and personnel immediately
lowered in unstable situations.
 All operating personnel must be familiar with the emergency controls and
emergency operation of the Manlift.
 Always ensure that power tools are properly stowed and never left hanging by
their cord from the platform work area.
 Safety Helmet and other mandatory PPE must be worn by all personnel.
 During operation, keep all body parts inside platform railing.
 Warn personnel not to work, stand, or walk under a raised boom or platform.
Position barricades on floor if necessary.
 Be sure that the ground conditions are able to support the maximum intended
load.
 The operator must be familiar with the surface before driving. Do not exceed the
allowable speed, side slope and grade while driving.
 Never exceed the maximum rated capacity specified by the manufacturer.
Distribute loads evenly on platform floor.
 Check work area for clearances overhead, on sides, and bottom of platform when
lifting or lowering platform, and driving.
 Keep non-operating personnel at least 6 ft. (1.8m) away from Manlift during all
driving and swing operations.
 Use of Manlift should be restricted during heavy wind, rain, sand storms and at
dark hours.
 Do not place boom or platform against any structure to steady the platform or to
support the structure.
 Do not operate any machine on which safety or instruction placards or decals are
missing or illegible.
 Sufficient Fire Extinguisher shall be provided in work place.
 Other precautions outlined in KNPC SWP of Scaffolds, Ladders & Work at
heights (HSE-TSSA-06-2015) and SWP Checklist to be followed.
 Stop the work if unsafe or potential hazardous condition found during the work.
4  Heat Stress  Drink sufficient cold water. Issuer
 Modify work schedules and arrange frequent rest periods with water breaks in 2 3 L Executor
shaded areas.
 If the worker is not alert or seems confused, this may be a heat stroke. CALL 888

IMMEDIATELY, Move the worker to a cooler/shaded area and apply ice as soon
as possible.

5  Poor hazard  Conduct Safety talk before starting the job and explain all precautions in Executor

 Explain emergency evacuation procedures to ensure workers can escape easily in
case of emergency. (Follow the wind sock cross/up wind direction to reach the
nearest transfer point).
 Ensure that workforce has understood all the precautions properly.
We agree with the residual risk levels and control measures required to perform the tasks based on the identified hazards. We confirm that we, the members of this
CTRA team have visited the job site to identify and document all the hazards associated with the activity.

Notes:
1. CTRA contains only those hazards and controls which are not captured in SWP Checklist and which could not be captured in Work Permits
2. Only competent (knowledgeable, skilled, trained & experienced) personnel {personnel who are competent to do this critical work} shall carry out this critical
work; This CTRA becomes invalid if this requirement could not be met
3. Implementation of all controls specified in Work Permits, SWP Checklist and CTRA is mandatory to ensure safe completion of this critical work
4. CTRA reference number shall appear in all pages of CTRA
5. Master Copy (original) of CTRA shall be retained in file for records and future reference
6. Photo copy of CTRA shall be attached with work permit
