    hereby depose and state:
    1. I Am the lawfully owner of the vehicle with Certificate of Registration
    number 21852033 and MV file no. 0244-00009511860.
    2. In the LTO system the details/information of the vehicle said are blank.
    3. I hereby certify and affirm that the blank on the details/information on
    my Certificate of Vehicle are stated below.
