    AFFIDAVIT

    affirm and declare that, I am working as Common Services Centers Village Level Entrepreneur CSC VLE
    having CSC ID _____________________________________ and running CSC centre allocated at address
    _____________________________________________________________________________________
    ____________________________________________________________________________________.

    I will follow all the mandatory Guidelines of CSC Scheme issued by CSCSPV and District Administration.

    I will update my Police Clearance Certificate (PCC) on CSC Digital Seva Portal on every 6 month or date of
    expiry, which is earlier from both.

    My CSC center has a proper set-up, CSC common branding board at center, display of authorized service
    rate-list, have CCTV camera and visitor register with record of customers (Service Name, Service Charge
    from customer, Name of customer, Date of visit, Mobile number of customer etc).
    I will deliver all the mandatory CSC services through my Digital Seva Portal such as PAN Card, IRCTC
    booking, Banking, Insurance, Digipay, National Pension Scheme, Grameen e store, etc and will try to do
    minimum 200 transactions every month.
    In case of non-operational Bank BC point than CSC has rights to take appropriate action against my CSC
    ID, either deactivation or disable of CSCID. As per bank directions I will do minimum 10 APY, 30 PMJJBY ,
    30 PMSBY per month.
    I will operate my CSC center from my allocated location only.
    If I will get Aadhar Kit, I will not move that to anywhere, else than allocated location.
    I am liable to pay all recoveries/Payments to CSC SPV against service delivery.

    Mandate services to be delivered by every CSC :-


    Digipay ( Pesnion Point ): Minimum 50 Transaction Per Month
    Electricity Bill collection : Minimum 30 Bills in every two Months
    Saral Transaction( If active) G2C: Minimum 50 Transaction Per Month
    PMJAY ( G2C): Minimum 30 Transaction Per Month depends upon your area Pendency
    Maandhan Schemes/Eshram ( G2C ) /PM Kisan : Minimum 10 Enrollment Per Month

    CSC-SPV reserves the right to deactivate my CSC ID in case of non-transacting for a period of three
    months.
    CSC-SPV reserves the right to deactivate my CSC ID and initiate legal action in case of over-
    charging/malpractice with the citizens.
    I shall extend full cooperation and settle all penalties (if any) incurred for any service against my CSC ID.
    I shall submit copy of my Aadhaar card and Family ID along with this affidavit and also disclose the
    information on number of CSC IDs taken in the name of my family members.
