    AFFIDAVIT OF LOSS

    I, JASMIN P. DAVID, of legal age, Filipino and currently residing at


    Sepung Barrio St., Brgy. Sta. Rosa, Concepcion, Tarlac, after having been duly
    sworn to in accordance with law, hereby depose and state: THAT-

    1. I am a holder of ATM Card issued by the RCBC, which I usually kept inside
    my wallet;

    2. My RCBC issued ATM Card was lost sometime on December 18, 2020
    when it accidentally fell from my wallet while I was travelling;

    3. I have exerted all efforts to locate my aforesaid ATM Card issued by the
    RCBC, but the same cannot be found anymore. Thus for all intents and
    purposes, the aforesaid ATM Card issued by the RCBC issued to me is
    considered lost;

    4. I am executing this affidavit to declare the fact of loss of my aforesaid ATM


    Card issued by the RCBC, to inform the authorities of the RCBC of Capas to
    the fact of loss, to secure the replacement of the same, and for whatever
    other legal purposes it may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 21 st day of


    December 2020, in Municipality of Capas, Tarlac, Philippines.
