    AFFIDAVIT OF INSUFFICIENT INCOME OF SOLO PARENT

    I, ______________, of legal age, single, Filipino


    and resident of __________________________________, after having been duly
    sworn to in accordance with law hereby depose and state THAT:

    1. I am the biological mother of ________________________________, an


    applicant for the Special Program for Employment of Students
    (SPES);

    2. I and the biological father of my child never got married, in fact


    our relationship terminated in the year 2001 and have never
    communicated ever since the separation;

    3. I was thereafter left alone with the sole responsibility of


    parenthood and in providing for the needs of my child;

    4. As a “Kasambahay”, I am a low-income earner;

    5. My monthly income is only Five Thousand (P5,000.00) pesos,


    derived only from my work as a “Kasambahay”;

    6. The said income is so meager and may not be enough to cover my


    family’s needs;

    7. No imminent change is expected in my financial status or


    employment during the next 12 months; and

    8. I am executing this affidavit to attest to the truth and veracity of


    the foregoing facts and to support my son’s application for the
    SPES.
