SHORT NARRATIVE REPORT:


1st Rotation
Since it was our first clinical experience, I was nervous. We were assigned to the
vaccination site on our first day, although Ma’am Grace, our clinical instructor reminds us that
we will inject a vaccine into real patients, it was really nerve-wracking but gladly, I was assigned
with Sir Chard and he guided me neatly and even taught me some proper techniques in other
types of parenteral medications. All throughout our experience with Ma’am Grace, it was really
spontaneous and fun not just to learn basic things but as well as how to properly save lives. On
our last day in the 4th-floor ward, Ma’am Anne let us perform IV insertion with each other and
prepare the medications for the patients that we are currently handling.
2nd Rotation
The D-day has come! Finally, our CI is Ma’am Kate, I know how strict she is when it
comes to our clinical duty. I never missed a day that she will notice a single mistake and I was
really thankful to her that I’ve learned basic standards procedures in so many things. Some
nurses who were alumni of our school said that in order for us to become future nurses Ma’am
Kate must handle us no matter what and finally I understand what they’re trying to convey. We
experience the best duty in our rotation, not because our CI is letting us do procedures that we
must learn but as well as we are establishing the great adventure in our nursing life and it is all
thanks to Ma’am Kate’s ways.

1st Rotation:
In our first rotation, I learned that mixing vaccines is not that easy, you need to be extra
careful with each vial, avoid being clumsy, and prevent yourself from needle prick.

2nd Rotation:
In our second rotation, our C.I taught us how to be standard in everything from tying our
hair to everything we need to make.

SHORT NARRATIVE REPORT:


1st Rotation:
In our first rotation, I learned about the basic chartings, vital signs, administering
medications through IV, and Basic Life Support. I even do a return demonstration on how to do
CPR to our Clinical Instructor, Ma’am Grace. I also get a chance to have a skin test and IV for
each of our partners.

2nd Rotation:
In our second rotation, I learned a lot by having to bathe a newborn baby, how to remove
a catheter, prepare medication, and return the medication ticket in proper order. Ma’am Kate did
put all her efforts into teaching us the standards of nursing.

SHORT NARRATIVE REPORT:


1st Rotation:
I was able to try to inject some people with a booster on the first and second days of my
first rotation. I also learned how to write on a patient chart and also how to perform CPR.

2nd Rotation:
In our second rotation, our Clinical Instructor, which is Ma'am Kate taught us the
standard and proper procedures within the hospital/ward from the start, so that when we become
nurses, we can take them with us wherever we go. She may be strict with the procedures, but I
know she just wants us to be the best nurses we can be in the future. I also learned some of the
hospital's safety precautions, such as how, in the event of a fire, you should turn off all oxygen
connected to your oxygenated patient. During an emergency, you should be familiar with the
hospital's horizontal and vertical evacuation procedures. You must keep track of the number of
your patients and employees. I also learned how to take a bath, and how to breastfeed and burp a
newborn baby. I also learned how to prepare medication and how to write on a patient's
discharge slip.

SHORT NARRATIVE REPORT:


1st Rotation:
In our first rotation, I was assigned vital signs every duty and one time I was able to give
meds and learned how to prepare them before handing them to the patients. One of the nurses
there also taught me how to make a discharge form and Ms. Grace our C.I taught us how to
perform proper CPR. Also, I was assigned to the HD, and the nurses there are so kind to teach us
the procedures and reasons to have dialysis.

2nd Rotation:
In our 2nd rotation, I learned a lot because Ms. Kate is our C.I. We are assigned to a ward
3rd floor. I learned how to remove a catheter in a proper way and performed it. I also gave
different meds to the patients and make a drug study. Ms. Kate also taught us how to correct our
mistakes in the chart and how to log them correctly. I learned how to bathe the babies and I was
able to perform the heplock in a proper procedure.

“To be honest, I had not planned on taking this course, but my aunt who lives in another country
has assured me that she will assist me if I decide to pursue a career in nursing. My hobby is all
about arts; I like drawing, painting, and designing things; therefore, at first, it was challenging
for me (and it still is challenging up to this day), but as time went on and as I gained more
experience through our on-the-job training, I began to enjoy this line of work more and more.

SHORT NARRATIVE REPORT:


1st Rotation:
In the first rotation, I'm not sure if I should be confident or not. We were put in the
vaccination area, and I was the first person to administer the vaccine. Although it was certainly
nerve-wracking, by the second day, we had gotten the hang of things and were more confident in
our abilities. We gained a lot of knowledge and experience from Ma'am Grace, including how to
give drugs, take vital signs, and track patients' progress.

2nd Rotation:
Since Ma'am Kate teaches the standard procedures, our second rotation is my favorite.
Ma'am Kate taught me several skills, including catheter removal, interviewing patient, health
education, charting, and medicine administration, and most importantly, I understood my clinical
strengths and shortcomings

SHORT NARRATIVE REPORT:


1st Rotation:
In our first rotation, we were assigned to the vaccination area. I’ve learned how to
vaccine the person properly and I was assigned in the area of how to dilute the Pfizer properly.
Second, we are assigned to the 4th floor, we take the vital signs, intake, and output of the
patients.

2nd Rotation:
We were assigned to the 3rd floor ward. First, we were assigned in taking the vital signs
of the patients and our C.I. taught us to remove the catheter of the patient and how to clean the
outside of the vagina before removing the catheter. Second, I and Ken were assigned in giving
medications to the patient, and also Ma'am Kate taught me to dilute the Piptaz so fast. The third
is I was assigned again to take the vital signs of the patient but I choose to take care baby patient,
I breastfeed him using the syringe and I burp him after I feed him.

1st Rotation:
We're assigned to the vaccination site. we take vital signs, interview, and also administer
Pzifer vaccines to individuals.

2nd Rotation:
Our CI Ma’am Kate, we did charting, vital signs, and many procedures. Ma’am Kate
taught us the proper ways how to prepare meds and how administer them. Many mistakes but I
learned from them.
