First Year

SGD CASE (MSK 1st Week)

INSTRUCTIONS

1. Read the case below, which will help us correlate clinically the topics in the basic sciences.
2. Concept guides are provided to assist you in which topics to study.
3. Please refer to the booklet on “Scored Group Discussion” by Richard Lander. We will be using
most of the elements discussed unless otherwise specified.
4. Length of time: 3 hours per meeting.
5. Time for preparation: 1 week prior to the schedule of the meeting.
6. Rotating roles of group members: leader, secretary, moderator
a. Leader: secures copy of the case, assigns roles, initiates case discussion
b. Secretary: checks attendance, lists ideas, writes down concept map for discussion
c. Moderator: keeps the flow of discussion on track and mediates arguments
7. Speak English during the discussion.
8. Present a concept map at the end of the SGD. The following link can serve as a guide for concept

LEARNING OBJECTIVES

1. Identify the different types of pain and their associated fibers.


2. Explain the mechanism behind the suppression of pain.
3. Understand the physiology behind referred pain.
4. Demonstrate understanding of pain pathways through a common clinical scenario, i.e., headache.

THE CASE

Josh is a 28-year-old male who consulted at the outpatient department of Urgello’s premier hospital for
headache. He states that it started just two hours prior to the consultation. The pain is located on the
right side, particularly involving the forehead and right eye. He noted the pain to be throbbing in character
and took paracetamol with no relief from the pain. Upon further examination, you noted that the
headache has been recurring monthly since he started his new job last year. The pain subsides when he
lies down but is aggravated by bright lights, especially whenever events with fancy names like Mundo
Granate are held at the coliseum a few houses away from his apartment.

After requesting additional diagnostic tests, you concluded that Josh has migraine headache. Treatment
was prescribed and instructions were given to minimize migraine attacks.
