    APPLY FOR BIRTH OR DEATH CERTIFICATE

    Name Inclusion, Corrections and Non availability of Birth or Death Certificate Procedure

    User can visit url https://ts.meeseva.telangana.gov.in/meeseva/login.htm

    Choose User Type as “Citizen” you will see “New User” option at down right. Click on “New
    User” and register filling required details. Your mobile number will be validated with “OTP”
    for registration. After successful registration come back to above link to login.

    Procedure for Applying a New Birth or Death Certificate:

    On Dashboard Select option “Certificates”. After Selection you will see department details.
    Select “CDMA” by clicking on the logo.

    Select Service “Birth or Death Certificate”

    Up on Selection fill the form. Select Service “Birth Certificate” for applying for a birth
    Certificate or select “Death Certificate” for applying for death certificate

    Enter “Registration Unit Id”. If you are not aware of the Registration id, please click on
    “Search” button side. Up on clicking “Search” new search window opens

    In new opened search window Select your district, location type, unit id and input various
    parameters such as date of birth, Gender, father name etc., based on search results select
    relevant record. If no record available click on “data not available-click here” link beside
    “Search” button

    After Selecting relevant record Click on “Get Details” details will be loaded on the form. Fill
    remaining required fields and make payment. You will receive a Application number and SMS
    confirmation on application

    When Certificate is ready you will receive SMS on mobile number and can download
    certificate

    Procedure for Applying other Birth & Death related Services:

    ➢ Child Name inclusion in Birth Certificate


    ➢ Correction in Birth Certificate
    ➢ Non-Availability of Birth Certificate

    ➢ Correction in Death Certificate


    ➢ Non-Aavailability of Death Certificate

    On Main Dashboard in Mee Seva login Please select “Application Submissions”. Under that
    Select “CDMA” by clicking on logo. Select required service and fill the form. Make payment
    to receive certificate.
