    AFFIDAVIT OF LOSS

    I , CORAZON HERALDINE N. RAMIREZ , of legal age, Filipino, single, resident of Mansasa District.,
    Tagbilaran City, Bohol, after having been duly sworn, hereby depose and state, that:

    1. I am the owner of BOHOL’S GREATMINDS-GREATFUTURE A TUTORIAL, TRAINING &


    ENHANCEMENT CENTER with business address at The Courtyard Bldg, Benigno Aquino Ave.,
    Cogon Dist., Tagbilaran City, Bohol;

    2. Last December 16, 2021, due to typhoon Odette that caused flooding, various documents
    on our archived were damaged;

    3. The BIR documents on archived could not be found despite diligent search and efforts;

    4. Original Certificate of Registration, Original Poster (“Ask for Receipt” & “Notice to the
    Public”), Original Books of Account, Inventory of Unused/Expired Receipts, BIR payment
    receipts and other documents were destroyed and/or lost;

    5. Diligent efforts were exerted to retrieve or locate the aforesaid documents and records but
    to no avail;

    6. I am executing this affidavit to attest to the truthfulness of the foregoing statements and
    for whatever legal purpose this may serve best.

    IN WITNESS WHEREOF, I have hereunto affixed my signature this 25 th day of January 2023 at Tagbilaran
    City, Bohol, Philippines.
