    CSC RO IX TO CONDUCT 19 JUNE 2022 CSE-PPT FOR THE GENERAL PUBLIC

    The Civil Service Commission Regional Office IX and its Field Offices in the cities
    of Pagadian and Dipolog shall conduct the 19 June 2022 Career Service
    Examination - Pen and Paper Test (CSE-PPT) for the Professional and Sub-
    Professional Level.

    Filing of applications shall be on a SLOT RESERVATION basis though the CSC


    RO IX Application Scheduler and shall run from 12 April 2022 to 28 April 2022.

    Due to the limited number of available schools to be used as testing venues, a


    quota on the number of applications to be accepted has been set.

    Filing of applications may, however, be CLOSED EARLIER than the set deadline
    upon reaching the set quota.

    HOW TO FILE FOR THE CSE-PPT

    1. Interested individuals are encouraged to start securing your appointment


    booking through the CSC RO IX Application Scheduler starting 08 April 2022,

    Moreover, the CSC Zamboanga Sibugay Satellite Office at ZSNHS Campus A,


    Pangi, Ipil, Zamboanga Sibugay shall accept applications exclusive only for
    the Zamboanga City testing center.

    Strictly NO CONFIRMED APPOINTMENT BOOKING, NO PROCESSING OF


    EXAMINATION APPLICATION. Be aware that only ONE APPLICATION is
    allowed for every CONFIRMED APPOINTMENT BOOKING.

    Failure to appear on the scheduled appointment shall result in forfeiture


    of the slot and the applicant shall be required to book for a new
    appointment.

    2. Bring the following requirements on your scheduled appointment;

    a. Electronic copy of confirmed appointment booking generated through


    the CSC RO IX Appointment Scheduler
    b. Properly accomplished Application Form (CS Form No. 100, Revised
    September 2016) Available for download at bit.ly/CS100
    c. Four (4) pieces of identical ID pictures with the following specifications:
     Passport size (4.5 cm x 3.5 cm or 1.8 inches x 1.4 inches)
     Colored, with white background
     Taken within three (3) months prior to filing of application
     Printed on good quality photo paper (i.e. photo does not peel off)
     In standard close-up shot (from shoulder level up with the head and
    face occupying at least 80% of the picture and with the name tag
    positioned at approximately 1 inch or 2.54 cm below the chin)
     In bare face (no eyeglasses; no colored contact lens; no headdress, no
    bandana, or any other accessories that may cover the facial features;
    facial features not computer enhanced)
     Showing left and right ears
     Taken in full-face view directly facing the camera, with neutral facial
    expression, and both eyes open
     With HANDWRITTEN (not computer-generated) name tag legibly
    showing SIGNATURE OVER PRINTED FULL NAME in the format -
    Given Name, Middle Initial (if any), Last Name, and Extension Name
    (if any)
    d. One original and photocopy of identification card which must be valid
    (not expired). The full list of accepted ID cards is available at
    bit.ly/cscacceptedids
    Note/s: All other ID cards not included in the list shall not be
    accepted. If the ID card does not indicate(d) applicant’s date of birth,
    original and photocopy of birth certificate issued by the Philippine
    Statistics Authority, or the Local Civil Registry printed on security
    paper shall be required.
    e. Properly accomplished Certificate of Consent available for download at
    bit.ly/csccoc
    f. Original and photocopy of Vaccination Card for fully vaccinated
    applicants. For unvaccinated/partially vaccinated applicants, a Negative
    RT-PCR/Antigen Test result shall be required on the examination day
    g. Examination fee of five hundred pesos (PhP500.00)
