    AFFIDAVIT OF SELF-ADJUDICATION

    I, ________________________, of legal age, single/married, Filipino


    citizen and a resident of __________________, Philippines, after having been duly
    sworn to in accordance with law, do hereby depose and state:

    1. That I am the sister and sole surviving heir of my brother


    ___________________, who died last ______________, without any last
    will and testament in _____________, evidenced by his Certificate of Death
    issued by the Office of the Civil Registrar, herewith attached and marked as
    Annex “A”, and made an integral part of this Affidavit;

    2. That my aforementioned deceased brother left a savings account at

    Philippine Currency. For reference, attached herewith is a copy of the
    deceased ______Passbook and marked as Annex “B”;

    3. That at the time of his death, my brother had no debts, liabilities or


    obligations to any persons, agency or institution;

    4. That pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby adjudicate
    unto myself the said __ Savings Account with account no. _____________
    and the money deposited in it which represents my lawful inheritance, and
    hereby files the same with the Register of Deed of _________ City with the
    request that the said adjudication be made effective without judicial
    proceeding as prescribed by the aforementioned provision of the Rules of
    Court.

    facts for the release of any other claim or benefit in relation to his death
