Edmond Public Schools Required Enrollment Documentation

EACH STUDENT WILL NEED ALL OF THE FOLLOWING ITEMS TO ENROLL IN EDMOND PUBLIC SCHOOLS:

1. Two (2) Proofs of residency – One from each category below:


State law states that a child’s residence is the school district in which the parent/legal guardian holds legal
residence. There can be penalties for falsifying residency.
a. A current lease agreement, contract on a home (where you will be residing within 45 days), warranty
deed statement, or mortgage statement.
b. Current utility bill (gas, electric or water); telephone and cable bills not accepted.

2. Legal birth certificate:


It must be the certificate issued by the state or country where the child was born – not the hospital record.

3. Parent/Guardian photo ID:


The biological, foster or adoptive parent may enroll the student. A driver’s license or any other photo ID is
acceptable.

4. Legal concerns:
Guardians must have current proper copies of court authorization. In cases of divorce, current custody
documentation is required, if applicable.

5. Official Transcript/Report Card:


Transcript (grades 9-12) or final report card (grade 8) AND/OR withdrawal grades if enrolling during a current
school year.

6. Immunization record:
The state requires acceptable evidence of up-to-date immunizations or a completed and signed exemption
form before a child may be enrolled in any school in Oklahoma.
