    accordance with law, depose and state:

    1. That I am the lawful owner of a motor vehicle which is more particularly

    4. That I am executing this Affidavit in order to inform the authorities concerned,


    particularly the Philippine National Police and Land Transportation Office relative

    existing color and for all its legal intents and purposes.
