    AFFIDAVIT OF LOSS

    I, ANITA P. MARISCAL, of legal age, Filipino, widow and a resident of 1071


    Estrada 6th Street, Zone-II, Digos City, Davao del Sur, Philippines, after having been
    duly sworn to in accordance with law, do hereby depose and say:

    That I am the same and identical person who caused for the execution of this
    Affidavit;

    That I am a purchaser and lawful owner of a certain memorial low located at


    Digos Memorial Park, Brgy. Tres de Mayo, Digos City, Davao del Sur;

    That as such, I was issued by Digos Memorial Park a Certificate of Ownership as


    proof;

    That I am the one who kept the Certificate of Ownership in my personal file;

    That just lately, I discovered that the same were lost and were no longer in my
    file where I have kept it;

    That despite diligent efforts, I have failed to locate the said Certificate of
    Ownership up to the present. I now consider them as irretrievably lost, for all legal
    intents and purpose;

    That I am executing this Affidavit in order to attest the truth of the foregoing facts,
    and to apprise the proper authorities of the same and for whatever legal purposes this
    may serve best;

    Further affiant sayeth naught.

    IN WITNESS WHEREOF, I have hereunto set my hand this 18th day of January
    2019 in Digos City, Davao del Sur, Philippines.
