Pre-Certification
The following events needs to be pre-certified by calling IMG directly:
• Interfacility Ambulance Transfer: No coverage if Pre-certification requirements are not met.
• Medical Evacuation: No coverage if not approved by the Company. Refer to the EMERGENCY MEDICAL
EVACUATION provision for complete requirements and coverage.

• If these treatment and supplies are not pre-certified: fifty percent (50%) reduction of Eligible Medical Expenses if Pre-
certification requirements are not met. Maximum Penalty: $1,000

Exclusions
Except as expressly provided for in the BENEFIT SUMMARY, all Charges, costs, expenses and/or claims incurred by the
Insured Person, and any claim for death or dismemberment benefits, and directly or indirectly relating to or arising or resulting
from or in connection with any of the following acts, omissions, events, conditions, Charges, consequences, claims,
Treatment (including diagnoses, consultations, tests, examinations and evaluations related thereto), services and/or supplies
