1.0 INTRODUCTION

This document covers method of the boiler steel structure erection


work, floor unit and parts of simultaneous construction.
The total weight of steel structure is approximately 10,300ton/Unit
(Tier1:3,100ton Tier2:2,400ton Tier3:2,400ton Tier4:2,400)
The simultaneous construction is one of the safety (reduce risk in high
places even in a small area) and rationalized method of an installation
of the boiler components, air duct, pipes and support with structural
steel erection at same time.
To achieve simultaneous construction, the following activities shall be
required.
1) All the materials and equipment which will be installed
simultaneously with each section shall be supplied to the site on
necessary timing.
2) Major items such as air duct, main piping etc. shall be pre-
assembled as large block. A size is determined in consideration of
an access road.
3) The support lugs for the piping and ducting support hunger shall be
set on the structural steel at grand. The hungers and rods shall also
be pre-assembled and set to the steel structure before lift (as much
as possible).
Floor unit means, block assemble of the floor which consist of a girder,
beam, grating and hand rail. Bolt joint of the structural steel shall be
100% tightened at the ground. (refer to Fig-1)
A size of unit is determined in consideration of an access road.
The specification of crane arrangement and tools for lifting are shown
in Attachment-6,7

2.0 TRANSPORTATION, UNLOADING AND STORAGE


2.1TRANSPORTATION

In principle, suitable transport such as trailers, low bed trailers, or


other types shall be Selected and used for transportation. Road survey
for transportation access shall be conducted by competent personnel.
Before erection starts, the steel structure supervisor should be
checking and marking all steel Structure materials are as per the
approved field erection drawings at laydown area for Construction
work.
Steel structures material shall be transported to construction area
from the laydown including the preassembling area by a trailer, truck
or fork-lift. The reasonable transportation methods must be uses to
ensure no damage to the steel structure.
The supervisor and riggers team shall use adequate cranes, lifting
tools and devices to lifting steel structure material from laydown and
put it on the trailer step by step with bigger items are in bottom and
then smaller items are on top side. All steel structure shall be fixed on
trailer by wires and Level-blocks.
Transport route to construction site shall be planned, cleared and
checked prior to trailer movement.
For large-sized transportation on construction road, leading car will be
arranged.

2.2 MATERIAL RECEIVING

Inspection shall be carried out for all materials received at site in


accordance with TJB56-L2-OFF-C-00-ITP-0020 Field Inspection and
Test Plan.
All materials shall be inspected visually for identification, quantity and
appearance. If materials are find any defect or un-matching after
checking, OSD (over, shortage & damage) report shall be issued, and
these steel structures shall be placed in a controlled segregated area
or posted marked so that repair materials can be identified easily prior
removal from the storage area.

2.3 MATERIAL HANDLING

Care shall be taken to prevent the surfaces of steel structure materials


from contamination and damage during materials handling and
transportation. The primer coated steel structures shall be handled
with sling belt in order to prevent damage to the primer coated of the
steel structures. (If the steel structures have been set of lifting lugs for
lifting; sling or wire rope with shackles shall be applied for handling)
When stacking and transporting steel materials, it is necessary to
insert protective material (wooden, rubber etc.) between the steel
materials and prevent damage. and, to prevent the loads from
collapsing, the state of securing must be surely confirmed before
driving

2.4 MATERIAL STORAGE

All steel structure material shall be stored in a suitable manner to


preclude mixing of different area or loss of spools identity. All steel
structures shall be stored in separated lay-down area or designated
area by the contractor, based on site condition and contractor shall be
able to judge for the suitable storage area not limited to temporary
storage purpose to facilitate field installation.
In any circumstances the materials stored in a lay-down storage area
will be requested to be drawn out by the contractor for field
installation requirement. The steel structure materials shall be stored
on the temporary supports such temporary supports shall be elevated
whenever is necessary.
At site all outdoor steel structure material (example: Colum, Beam,
Bracing, plate, etc.) shall be placed on wooden block, sleeper or frame
minimum 100mm above ground to avoid damage/dart from rain water.
Bolts/nuts/Washers shall be put in box and kept inside container or
covered with PVC sheeting should be fitted.

3.0 ERECTION PROCEDURE

3.1 BEFORE ERECTION

The following checks are to be carried out,

a) Geometric layout

b) Checking bench mark (coordination)

c) Checking boiler center line

The reference centerlines of the boiler steel structure shall be


marked on the surface of the boiler foundation. (refer to Fig-2)

After the marking is placed, perpendicularity of the intersecting


reference centerlines shall be checked.

The reference centerlines of the boiler shall be determined and then


marked by measuring the dimensions A and B,

tolerance ±1mm.

The centers of the columns shall be determined and then marked by


measuring the dimensions a, b, c, d, e, f, g, h, i,

tolerance ; AISC standard, target ;±3mm

d) Checking anchor bolt position and elevation

The center and level of the anchor bolts shall be checked, and the
relation between the center of anchor bolts group and center line of
column shall be measured and recorded.

e) Checking foundation level and size of column

On carrying the above checks with respect to the approved drawing


the Surface preparation i.e. chipping and cleaning is performed on
the foundation.

3.2 PACKER PLATE (SOLE PLATE) INSTALLATION

The following checks are to be carried out,

a) Check level of foundation.

b) Cleaning of chipping surface free from foreign particles.

c) Pre-soak the area to be installed.

d) Prepare and mix padding mortar with water as per the padding mortar
Manufacturer’s instruction.

e) Set the packer plate to the required level by using the grout & spirit
level. (refer to Fig-3)

The sole plate shall be set so that the top surface is truly horizontal
and on the design elevations with a tolerance of +0/-3 mm when
grout has become solid.

After grout has set, the elevation of the top surface of the sole plate
shall be re-checked

3.3 ERECTION OF STEEL STRUCTURE

3.3.1 Erection

a) Centre Line on Foundation and Column base plate shall be marked.

b) Generally, erection of steel structure must always be done by


making a square in order to achieve stability.
The general flow is refer to item 3.4 and general sequence of steel
structure is Attachment-1.
c) In areas BKL/BKR, BNL/BNR, L/R up to forth (4) tier, the main part
of columns, girders, connecting beams, bracings are installed
before lifting the top girder.
un-installed members would be decided as to the plan of exclusion
members which are in the obstruct of the products bringing in and
lifting such as duct, piping and panels or assembling equipment
and others.
d) After mounting of the main structural members is completed up to
forth (4) tier, lift and set the Top Girder of each row.
The top girder of row (BD), (BE) is lifted by the crane, and row
(BF) is planned to be lifted by jack system.
e) if assembled large block of column, beam, girder and other
members, temporary scaffolding shall be installed on the block to
release wire sling. Temporary scaffolding shall be complied with
project requirement, the job shall be done by qualified scaffolding
workers.
f) After the column is erected on the foundation, temporary guide
wires to be installed to fix with fixed structure or concrete block etc.
for safety purpose. Under the conditions of the structure that will
not collapse by the beam connection, will be released wire.
g) Generally, crane work will be suspended, if the average wind
speeds in 10 minutes exceed 10meters per second.

3.3.2 Checking

a) After the erection of the column of 1st tier shall be checked by


comparing the level marks placed on column. (Refer to Fig-4)

b) The location of the columns base shall be checked by the intersect


centerline scribed on the foundation. (Refer to Fig-5)

c) Verticality of the columns shall be checked by a theodolite and


adjustments can be made by tightening or relieve the temporary
guide wires. (Ref. Fig-6)

d) After checking of column alignment and beam interconnection
work, the TC bolts should be tightened by using shear wrench.

e) After checking of column alignment, the anchor bolt should be


tightened, and welding of square washer to fix completely

3.5 INSPECTION ACTIVITY

All the items stipulated in this section shall be inspected and


confirmed to be set within specified criteria. And the result of
inspection items shall be recorded to show the erection work to be
properly executed in accordance with field ITP.

Depending on the site situation, erection procedure and allowable


installation tolerance may have to be changed if Lot2/SCBD designer
and/or engineer considers it necessary.

a) Visual Inspection of Materials for Appearance

b) Base plate & foundation checking.

c) Centreline of boiler, and reference to column foundation.

d) Checking of datum or reference point.

e) Anchor bolts position Column foundation level.

f) Alignment, that Column is Plumb, Level

g) Anchor bolt & bolt tightening inspection

h) Grouting inspection.

i) Welding and NDE

j) Final inspection.

The work and inspection will be performed based on the following

4. SQUENCES OF ERECTION

Boiler steel structure will manage of products, assembly by the


following area division (Refer to Fig-7)

4.1 INSTALLATION

Erect structure the main member column, beam or girders and bracing.
After setting the assemblies tighten anchor bolts nuts and install
temporary guy wires to hold and stabilize the assembly. Note that
before erection joint surfaces are cleaned and assured no foreign
materials such as oil, soil, sand, etc. are present. Erect the next
assemblies doing the same steps.

Erect connecting beams girders, and bracings to square off the


structure. Repeat the same on all areas. To check and adjust the
verticality (plumbness) of columns and to align square off the
structure check the column plumb adjust by means of chain blocks or
turn buckles until it is within tolerance for erection. Tighten up the
bolts will be done after alignment inspection.

Stairs, handrails, Gratings are planned to be installed in parallel with


the construction of the steel structure as much as possible. Also, since
the main stairs are installed independently of the boiler main structure,
construction will be done by block method in parallel with steel
structure.

These operations must be adjusted according to safety standards,


such as lifting work, and work shall be done in condition that can work
safely.

All field welding works when shown in drawings will be done in


accordance with welding procedure specification.

4.2 BOLTING WORKS

Bolting works will be done in accordance with the requirement of the


materials (TC Bolt; S10T) specification.

4.2.1 Storage

a) Store in a place that is not exposed to rain and dust.

b) Aggregate for each size and manage it appropriately so as not to


be damaged by load collapse etc.

c) Do not mix bolts of different types and lengths in one box.

4.2.2 Surface preparation of connection

a) The joint surface of the steel structure will be delivered by primer


coating

b) If oil or foreign substance is attached to the joining surface and/or
the splice plate, it shall be cleanly removed.

4.2.3 Bolting & Tightening

a) Confirm steel structure alignment and use tapered wedge to adjust


bolt holes.

b) Install TC bolts of correct size progressively as stipulated in


specifications.

c) First tighten the bolts using manual wrench. (Refer to Fig-8)

d) Using electric shear wrench systematically progress tightening


from the most rigid part of the joint to its free edges until the end
part of the bolt is sheared off.
Before tightening work, make sure that the electric wrench is
adjusted to proper torque.
e) Appropriate ladder or scaffolding will be installed and used to
access to the bolt joint that cannot be tightened the bolts from the
steel frame on floor. Generally, Those items will be provided by
steps and lugs for safety wire for climbing and safety work on the
column and vertical braces shown in attached-8 (reference)
f) When tightening of the bolt is completed, sampling inspection shall
be performed to ensure that each connection part is tightened.
4.3 Touch up painting/Finish painting Works
Painting works will be done according to the following painting
Specification.

4.4 Grouting Works

Grouting of column base plates will be done after completing erection


of tier 2 steel structures as per below grouting procedure.

a) Grouting work under the base plates for the main columns shall be
done after first tier steel structure erection and before 3rd tier
steel structure erection finishing.

b) Cleaning of chipping surface area, pipe sleeves to free from foreign


particle and water

c) Prepare formwork and Pre-soak the area to be grouted in


compliance with Manufacturer’s instruction.

d) Prepare and mix the non-shrink grout with water as per the
attached Manufacturer Instruction.

e) Mixing of grout can be done by using mechanical mix with low-


speed.

f) Mix to a uniform consistency minimum of 2-5 minutes.

g) Finish the surface with a trowel.

h) After completion of the grouting, cover the exposed portions with


such material as hemp cloth and keep maintain the moist condition
for curing. The minimum curing time is 7 days per Manufacturer
Instruction.

i) Grouting of major structure column beams above 9m will be


performed using Non-Shrink grout and minor structure below 9m
will be done with ordinary cement as per technical specification.

4.5 BOILER & BUNKER STEEL STRUCTURE

The overall construction sequence is divided into 5 areas namely;

BKL/BKR, BNL/BNR, L/R, AFL/AFR, AHL/AHR. (Refer to Fig-7)

A. 1st-Tier (FL.0 to SEL.23.700m).

a) Erection of column on BA B54-57, BB B54-57.

b) Erection of column on BA B51-53, BB B51-53.

c) Erection of floor beams and Bracings up to EL +11.000mtr on
BNL, BNR, L, R areas.

d) Erection of column on BNL, L BC56-57 to BG56-57.

e) Erection of column on BNR, R BC51-52 to BG51-52.

f) Temporary install of Primary air & Tempering air duct at


EL+11.000 between area BNL,BNR, L,R.

g) Erection of floor beams up to EL +22.200mtr BKL, BKR, BNL, L,

5. RISK ASSESSMENT

5.1 Risk Assessment for steel structure erection

Refer to the attachment-1 of risk assessment sheet

5.2 Method Statement “ Work at Height “ of steel structure erection

1) General Requirements

When working at high, if there is any risk of fall off from the girder
or beam, near the opening, from outside of the handrails or
scaffolding a full body safety harness must be worn, and where a
fall exposure of 1.8 m or more exists, maximum use will be made of
primary fall prevention systems such as scaffolding, barricading,
personnel hoists and man lifts.

Ancillary fall protection equipment such as static lines, perimeter


guards or other suitable means of anchoring safety harnesses, will
be utilized by personnel traveling from one location to another in
elevated locations where there is a potential to fall.

Where personnel lifting devices (man lifts) are being used the
occupants will wear full body harnesses and be hooked up to a
secure point within the equipment. When a crane suspended
personnel basket is used to access an elevated position a self
retractable lifeline will be connected to the crane rope above the
headache ball using a shackle. The occupants of the personnel
basket will connect their fall arrest shock absorbing lanyard to this
lifeline.

Movement of these personnel lifting devices whilst occupied will be


limited to operations essential for carrying out the work.

Access to structural steel will be by use of scaffolding, ladders, man


lifts or other approved personnel hoisting devices.

Attachment-8 shows examples of safety facilities such as step, lug


equipped to the main column and large-size braces etc. and these
shall be maximum utilized.

2) Fall Prevention

Normally, there are many conditions that cannot be installed on the


floor and the walkway in the process of construction stage, and
should be work at the height location. Therefore, taking into
consideration the work and access after lifting the beams, you will
install the lifeline or temporary scaffolding before lifting and
assemble the steel structure. Refer to below for reference.

Horizontal Lifeline

"Lifeline" means a component consisting of a flexible line for


connection to an anchorage at one end to hang vertically (vertical
lifeline), or for connection to anchorages at both ends to stretch
horizontally (horizontal lifeline), and which serves as a means for
connecting other components of a personal fall arrest system to the
anchorage.

Lifelines will not be used for any purpose other than fall
protection

Lifelines will be a minimum of 10mm wire rope or 16mm nylon


expect hot work area, or equivalent.

Lifeline will be conducted a visual inspection prior to their use.

Even during bolt tightening work after lifting, you will also work with
these lifelines. However, the underside of the beam that cannot be
worked from the top will do the work using a temporary scaffolding
or step etc.

Retractable Lifeline

Prior to and during lifeline or scaffolding placement, personnel will


crawl (coon) steel members with shock absorbing lanyards secured
around said steel members. Retractable lifeline systems secured at
elevations above the operation will be used as required.

Retractable lifelines are devices that will serve to stop the free fall
of a person prior to that person striking a lower surface.

The retractable device will be attached in such a manner that it


does not interfere with the person who is using the ladder for
access and egress

• Retractable lifelines will also be used to provide a fall protection


system for structural steel erectors during erection prior to the
installation of other fall prevention and protection systems

• Retractable lifeline systems will be inspected quarterly by a


competent person.

• Retractable lifeline devices will only be installed by competent


persons

• Retractable lifelines will be attached directly to the full body


harness

CONSTRUCTION TOOLS & EQUIPMENT INSPECTION PROCEDURE (Lot2)

1. INTRODUCTION
The construction tools and equipment inspection procedure shall focus on personal
safety in regard to the safe use, operation and inspection of equipment, tools,
vehicles and any other requirement utilized on the project site. Inclusive of ;
- Scaffolding
- Fall protection and prevention
- Heavy equipment
- Vehicles
- Lifting tools
- Electrical equipment

Initial and quarterly inspections shall be conducted by designated competent


persons and inspection records documented and maintained.

2. PURPOSE
The procedure is intended to protect the health and safety of personnel on site, to
manage, eliminate or reduce the potential for loss of life, injury, damage to the
environment or to assets throughout the duration of the TJB project.

3. SCOPE
This procedure shall form an integral part of the HSE management plan and
reference applicable to specific local regulation. (if applicable)
The procedure shall identify requirements regarding safety inspection and shall
require Lot2 contractors and sub-contractors align their own activities to ensure
compliance with the procedure.
The procedure shall detail inspection criteria for;
- Initial and quarterly inspection report
- Operators daily inspection check

The procedure shall apply to Lot2 project area, facilities and laydown areas.

4. INSPECTION
4.1 General Requirements
Tools and equipment utilized on the TJB project shall be subject to the initial and
quarterly inspection and color cording process.
The timing and color cording of tools and equipment inspections shall be uniform
across all contractors throughout the project site, as follows:
Inspection color cording
JAN.01 to MAR.31 RED

APR.01 to JUN.30 GREEN

JUL.01 to SEP.30 BLUE

OCT.01 to DEC.31 YELLOW

Once inspected and approved for service, equipment will be assigned a unique
inspection number. The details of the equipment will be entered in an equipment
inspection register and a corresponding tag or sticker, of a durable plastic or similar
material, depicting the current color code will be affixed to the equipment.
Whenever the internal inspection schedule date is interfering with the local law’s
inspection date, then priority will be given to the inspection of local law regulations.
In such case that particular inspection date of contractors schedule may be skipped
and local law may be followed.
Equipment found to be defective (for inspection criteria) may be re-presented for
inspection after the identified defects are repaired.
Contractor may commence re-inspection of tools and equipment seven days prior to
the end of current inspection quarter and extend through to seven days after
commencement of the succeeding quarter. During this period either color code shall
be acceptable for use. Tools and equipment not inspected shall not be accepted for
use on the project after the seven days “grace” period.

4.2 Tools and Equipment Inspector


HSE manager will be assigned as equipment and tools inspector. The inspector shall
be responsible for inspecting and certifying as fit for purpose of the equipment and
tools. For electrical equipment, electrical engineer and electrician shall be in charge
of electrical equipment inspection.

5. INSPECTION AND DAILY CHECKLIST


Inspection forms and checklists required by this procedure are available from HSE
department. Contractors may issue inspection forms and checklists through other
sources but such forms and checklist shall ultimately be approved by HES manager.
