AN OVERVIEW
Sanjay Gandhi Postgraduate Institute of Medical Sciences (SGPGI) Lucknow is a university established under an Act of the Legislature of the State of Uttar Pradesh in 1983. The Institute is located on a 550-acre residential campus on Raebareli Road, of Lucknow. The Institute is rated among the top medical institutions in the country. Admissions to various courses are made once in a year in July and for appointment for Senior Residents (Hospital Services) twice in a year (January & July) through an all-India entrance examination held in the month of June & December (for July session on 1st Sunday of June (Theory) and for January session on 1st Sunday of December (Theory) every year). Detailed Advertisements based on the number of vacancies available in each department are put on Institute web site www..sgpgi.ac.in.

ELIGIBILITY CRITERIA
Nationality: Only bonafide Indian nationals are eligible to apply. Qualifications: Candidates must possess requisite qualification recognized by Medical Council of India as on 31st May for July session and 30th November for January session. Essential qualifications for each program are given separately in detail chart of essential qualification on page 4 & 5 of the prospectus.

Essential Qualifications for DM/M.Ch Courses: MD (Medicine/Pediatrics) for DM courses. For Medical Genetics candidates with MD (Medicine/Paediatics) or MD/MS (Obst. & Gyne.) may also apply. Candidates must possess MS in Surgery for M.Ch courses recognized by Medical Council of India for this purpose. Age: Upper age limit for eligibility is 35 years on the first day of the month of start of the session (January/July). Age relaxation of a maximum of 5 years may be given to SC/ST/OBC candidates having domicile of Uttar Pradesh and to widowed/divorced/legally-separated women who have not re-married and disabled/handicapped persons as per Government rules. There is no upper age

Reservation: There is no reservation in DM/M.Ch/PDCC/PDF seats for any category. For Senior Resident (Hospital Services) reservation will apply as per rules. Reserved category candidates having domicile other than Uttar Pradesh will be treated as General category candidates for all purposes. Reserved category candidates should produce authentic certificate of their category, issued by a competent authority. Sponsored Candidates: Admission to various courses is open for candidates sponsored by Central/State Government Organizations, Universities and Defence services, subject to the following conditions. (a) Sponsored candidate will not be given relaxation in method of selection, duration of course, practical training, pattern of examination and payment of prescribed fee etc. except relaxation in age which will be as per rules. (b) SGPGI will not pay any salary, stipend or any other emoluments during the entire training period to the sponsored candidates (c) The candidate should provide a certificate from the sponsoring authority stating that: (i) The candidate is a permanent employee of the sponsoring organization. (ii) She/he will be granted leave for THREE years in case of DM/M.Ch and ONE year in case of PDF/PDCC programs.

(iii) The candidate after completing the course will be suitably employed by the sponsoring organization to work in the specialty in which the training is provided. (iv) The sponsoring authority shall be responsible for payment of salary or any other emoluments to the candidate during the period of training. (d) Sponsorship certificate should be attached with the application form, failing which the sponsorship will not be entertained.

HOW TO APPLY: Information brochure & Application form:

The application form along with information brochure can be downloaded from the Institutes website [http://www.sgpgi.ac.in] during 1st May 2011 to 20th May 2011. The candidate should read the instructions very carefully before filling up the form. The candidates will have to fill up the APPLICATION FORM ONLINE. After online submission, a print out of filled-in application form MUST be taken and sent to Executive Registrar, SGPGIMS, Rai Bareli Road, Lucknow -226014 along with all relevant documents in support of age, qualification, proof of recognition for MD/MS Degree from MCI, experience, reservation etc and duly signed by the candidate. The last date of receiving of completed application forms along with necessary enclosures is 20th May 2011. Candidature will be accepted only of those who will apply through online and also send a hard copy at the above mentioned address of the Institute.

APPLICATION FEE:
The candidate will be required to enclose a bank draft of Rs. 1000/- drawn in favour of Director, SGPGIMS, Lucknow Academic Account, payable at SBI, SGPGI Branch, (Code No. 7789), Lucknow, which is non-redundable.

CHECKLIST:
Please complete and arrange your application in the following order and firmly tag before dispatch to the Institute. (a) Bank draft for Rs.1000/- drawn in favour of Director, SGPGIMS, Academic Account, payable at SBI, SGPGI Branch, (code No. 7789), Lucknow. (b) Print out of the online application form in 3 Pages (2 page form and 1 page Identification cum Attendance card) with your signature at two places (page 2 of 3 and on Identification cum Attendance card page 3 of 3) and paste self certified passport size recent coloured photograph at the appropriate places (c) A self-certified copy of matriculation certificate/age proof certificate. (d) A self-certified MD/MS/ required passing certificate for Senior Residents and DM/M.Ch for PDF. (e) Candidate must submit copy of Medical Registration from Medical Council of India or State Medical Council / proof of recognition of his/her degree from Medical Council of India or with State Medical Council, failing which application will be rejected. (h) If employed, candidate must ensure that employers signature is at Page 2 of 3 of the application form (i) Please use your correct email-id and password while filling your online application form. It is your responsibility to remember your email-id and password. No telephonic /personal request, in this regard, will be entertained. (j) Subject code is provided in Appendix I and State code in Appendix II. (k) Application fee once paid is not refundable either in full or in part under any circumstances.

HOW TO KNOW YOUR ELIGIBILITY: Eligibility status of each applicant will be placed on the Institutes website on 01st June 2011 for DM/M.Ch/PDCC/SR (HS) and for PDF on 11th July 2011. HOW TO GET YOUR ADMIT CARD: Eligible candidates can print their Admit Card ONLINE (only once) between 01st June to 05th June 2011 and PDF candidates between 11th to 15th July 2011 using their email id, password and registration number written on right corner of application form HOW TO KNOW YOUR RESULT: The list of eligible candidates called for departmental test will be displayed same day following the PART-A objective written examination on the Institute Notice Board at Administrative Block. PARTB examination will be held in the concerned department of the Institute next day of PART-A examination

The final result of entrance exam will be available on the Institute website /
Institute notice boards within one week of the entrance exam. PROCEDURE FOR RE-EVALUATION: In case any candidate desires to re-evaluate his result, he/she will be required to submit an application in writing along with a fee of Rs. 3000/- via demand draft. The result will be made available in one week after the request is made. No request for re-evaluation will be entertained after one week of publication of the result.

IMPORTANT INSTRUCTIONS :
A candidate can apply for only one program. No change in course applied for will be allowed under any circumstances. However, candidates applying for DM/M.Ch courses may be considered for the posts of Senior Resident (Hospital Service) in the same speciality, if he/she qualifies for the same.

The completed Application Form and Identification- cum- Attendance Card along with relevant certificates should reach the destination on or before the last date as specified in the advertisement. Incomplete applications or applications received after due date shall not be considered and no communication in this regard shall be entertained. The institute takes no responsibility for delay or loss of application in postal transit. The not-eligible candidates may request for review of their applications to the Executive Registrar of the Institute between 1st to 3rd June 2011 after paying prescribed fee of Rs. 500/- through Bank Draft drawn in favour of Director,

SGPGIMS, Lucknow Academic Account, payable at SBI, SGPGI Branch, (Code No. 7789), Lucknow. However no additional document or information
other than submitted in the original application form will be considered for review. Request for review in any case will not be considered beyond the specified dates as above.

Candidates in employment must get their applications forwarded through proper channel. Subject codes are provided in Appendix-I and state codes in Appendix-II. The examination fee is not refundable either in full or part under any circumstances.

METHOD OF SELECTION:

The Entrance Examination will have two components (total 100 marks). Part: AObjective type MCQ written examination of 60 marks. It will be of 90 minutes duration and will consist of 60 MCQ based TRUE/FALSE questions. Each question consists of a stem and five statements. The candidate is required to mark each response as either true or false. All questions carry equal marks. One mark is awarded for each correct response and half mark is deducted for each incorrect response; no marks are awarded if no response is given. Candidates must use Black or Blue ball point pen to mark the responses. Based on the result of Part:A examination, candidates will be called for Part: B departmental objective examination in the ratio of 1:3 candidates per seat. Part: B- Departmental objective test of 40 marks at the level of HOD on the topics of patients/disease in the ward/laboratories along with other faculty members.

Candidates applying for PDF are exempted from theory examination, but they would have to appear in an interview to be held on Friday dated 15.07.2011 in respective departments.

MEDICAL EXAMINATION :
No candidate will be permitted to join a course, unless declared medically fit by the Medical Board constituted by the Institute for the purpose. The decision of the Board shall be final.

JOINING:
If a candidate fails to join the course by the date stipulated in the selection letter, the offer shall automatically stand cancelled and the seat will be offered to candidates in the waiting list in the order of merit. At the time of joining, the selected candidates shall have to submit their original Matriculation certificate, MBBS Mark sheets of all professional examination, MBBS Degree, Permanent Registration Number, MD/MS Passing certificate, Proof of MD/MS Degree recognition from Medical Council of India, Migration Certificate and Certificate of good conduct.

The fees will be paid in January and July each year between 1st to 10th day of the month. Fee for the first year will be payable at the time of joining and that for subsequent years within 15 days of renewal of appointment. The candidates will not be permitted to join without payment of the requisite fee. Salary will be released only after proof of payment of fee and completion of other formalities.

DUTIES AND RESPONSIBILITIES:


Students admitted to DM, M.Ch and PDCC courses shall simultaneously work as Senior Residents. They will be required to perform such work as may be assigned to them and are necessary in the legitimate interest of patient care, teaching, training, research and in Emergency services as and when required.

DURATION OF APPOINTMENT :
All Senior Residents (including DM/M.Ch candidates) are appointed for a maximum period of 3 years subject to renewal after the satisfactory performance report from their departments at the end of every year. If Extra ordinary leave for more than three months/maternity leave is taken by DM/M.Ch. students, the tenure of training will be extended for another six months with salary and examination will be held as per rules. Since the total tenure of SR (HS) is 3 years, the candidate is required to declare the period of training or service undertaken in other Institution as Senior Resident at the time of applying.

SALARY:
Senior Residents shall be paid salary in the pay band 3 of Rs. 15,600 39,100 with Grade pay of Rs. 6600 plus other allowances as per rules during his/her first year term, which will be increased as per rules during subsequent years. PDF candidates are paid salary equivalent to the 3rd year Senior Resident. Medical Physics Resident shall be paid salary in the pay band 2 of Rs. 9300 34800 with Grade pay of Rs. 4200 plus other allowance as per rules.

HOSTEL ACCOMMODATION:
All Senior Residents are required to reside in the campus. They are provided semifurnished hostel as per policy of the Institute.

Candidates selected for DM/M.Ch courses will have to execute a bond at the time of joining. As per terms & conditions of the bond, if a candidate discontinues the course without completion, he/she will have to pay a sum of Rs.1,00,000/- (Rs. One lac only) to the Institute.

GENERAL INSTRUCTIONS:
The date, time and venue of examination will be provided on admit card. No TA/DA shall be paid for appearing in the entrance examination or for joining. Number of seats/posts indicated in the advertisement against various programs is provisional and subject to change without prior notice. Any attempt on the part of a candidate to suppress facts, provide false information or influence selection directly or indirectly will be treated as a disqualification, and if selected, the services are liable to be terminated. Each selected candidate shall have to pay the fee/dues within the prescribed period, failing which his/her admission/appointment shall be cancelled. The selected candidates shall submit photocopies of medical registration, date of birth certificate, certificates of educational qualification, experience and relieving and good conduct certificate from the University/Institution last attended at the time of joining. The training programs are full-time and continuous, and private practice in any form after joining the Institute is strictly prohibited. All Senior Residents admitted to the Institute shall maintain good conduct, attend their classes regularly and abide by the regulations/guidelines of the Institute failing which their names are liable to be struck off the rolls of the Institute. Participation in any agitation, protest, strike, etc. is prohibited. Candidates not abiding with these provisions are liable to be terminated. Ragging is strictly prohibited. If any incident of ragging comes to the notice of the authority, the concerned student shall be given liberty to explain and if his/her explanation is not found satisfactory, the authority would expel him/her from the Institute. No intimation to non selected candidates will be sent and no correspondence on this subject will be entertained. These rules are subject to change in accordance with the decisions of the Institute, taken from time to time. The decision of the Director, SGPGIMS, Lucknow, shall be final in the matter of selection of candidate for admission to the various courses and no appeal shall be entertained on this subject.

FORCE MAJEURE:

If the entrance examination is postponed or cancelled (in part or in full) due to any reason whatsoever beyond the control of the organizers (viz. elements of nature, force majure or orders/directives issued by any competent government authority), the

institute shall not be responsible for any loss or injury sustained by the candidates directly or indirectly.
