    to in accordance with law, do hereby depose and say:

    1. That I am the direct and surviving heir of the late


    ________________, who during his lifetime was my registered electric co-
    consumer at the Bohol I Electric Cooperative, Inc., (BOHECO-I) for a
    certain residential building, situated at Laya, Baclayon, Bohol, under
    Account No. _____________;

    2. That by virtue of the foregoing statements, and being of sound mind


    and disposition, I hereby waive any and all rights over the above-mentioned
    electrical connection at the BOHECO-I in favor of the Vendee,
    _____________________, Filipino, of legal age, married
    ______________________, and a resident of Doljo, Panglao, Bohol,
    Philippines;

    3. That I am executing this affidavit without due force and


    intimidation, with full understanding of the consequences of my actions;

    4. That I am executing this affidavit in order to relinquish whatever


    rights I may have in the aforementioned electrical connection and for
    whatever other legal purposes this may serve.
